基于“互联网+”信息化医护平台造口患者延续性护理服务模式的构建及应用

摘　　要：目的基于“互联网+”信息化医护管理平台,构建造口患者居家延续性护理服务模式,并评价其应用效果。方法选取2021年1月至12月在“互联网+”信息化平台下单的居家造口护理患者(试验组)和同期出院患者(对照组)为研究对象,对照组采用常规造口住院服务流程,试验组在此基础上应用“互联网+”信息化平台进行居家造口延续性护理。比较两组患者造口健康知识知晓率、非计划再入院率和满意度的差异。结果与同期出院患者相比,接受造口延续性护理服务后,试验组患者对造口健康知识掌握率明显提高、非计划再入院率降低、患者满意度明显提高。结论“互联网+”信息化管理平台为造口居家患者提供系统、科学、专业的延续性护理服务,提高造口居家患者造口相关知识掌握情况,降低非计划再入院率,提高患者满意度,但还需要进一步形成较为系统、完善的居家护理服务评价系统和评价指标。Objective To construct a home continuous nursing service model for patients with ostomy based on the"Internet+"informa-tion medical management platform,and to evaluate its application effect.Methods Patients with home stoma care who placed an order on the"Internet+"information platform(experimental group)and patients discharged during the same period(control group)from January to Decem-ber 2021 were selected as the research objects.The control group received routine ostomy hospitalization service process,and the experimental group applied the"Internet+"information platform for home stoma continuous care on the basis.The awareness rate of stoma health knowledge,unplanned readmission rate and satisfaction were compared between the two groups.Results Compared with the discharged patients during the same period,after receiving the colostomy continuous nursing service,the mastery rate of colostomy health knowledge of the patients in the exper-imental group improved significantly,the unplanned readmission rate reduced,and patient satisfaction increased.Conclusion The"Internet+"information management platform provides systematic,scientific and professional home nursing services for patients with stoma at home,improves the mastery of stoma-related knowledge of patients with stoma at home,reduces the rate of unplanned readmission,and improves patient satisfac-tion.However,a more systematic and perfect home care service evaluation system and evaluation indicators need to be further formed.
