妇科肿瘤患者围手术期ERAS中西医结合康复护理实践方案的构建

摘　　要：目的 通过构建科学、规范、实用的妇科肿瘤患者围术期加速康复外科(ERAS)中西医结合护理实践方案,为促进妇科肿瘤患者术后康复提供参考。方法 以复旦循证护理实践路径图为指导,运用文献回顾法、结合我院临床中医护理常规初步形成妇科肿瘤患者围术期ERAS中西医结合康复护理实践方案,运用德尔菲专家咨询法进行2轮调适,征询专家意见,确定方案策略。结果 2轮函询问卷有效回收率为90%和100%,专家权威系数为0.81和0.85,两轮肯德尔和谐系数为0.162~0.258,三级条目变异系数范围在0.00~0.190,最终形成涵盖4个Ⅰ级指标,25个Ⅱ级指标,76个Ⅲ级的妇科肿瘤患者围手术期ERAS中西医结合护理实践方案。结论 所构建的妇科肿瘤患者围手术期ERAS中西医结合康复护理实践方案涵盖了妇科肿瘤患者围术期的中、西医护理策略,较为科学、全面、实用,可以为促进妇科肿瘤康复护理发展提供参考。Objective To construct a scientific,standardized and practical perioperative ERAS rehabilitation nursing practice protocol which integrated traditional Chinese and western medicine for patients gynecological with tumor,to provide reference for promoting postopera-tive rehabilitation.Methods Using the Fudan Evidence-Based Nursing Practice Pathway Chart as a guide,a literature review was used to form a preliminary perioperative ERAS combined Chinese and Western medicine rehabilitation nursing practice plan for gynaecological oncolo-gy patients,combined with our hospital's clinical Chinese medicine nursing routines.Through two rounds of Delphi expert consultation,the ex-pert opinions were sought to determine the plan strategy.Results The response rate was 90%and 100%for two rounds of expert consulta-tion,the expert authority coefficient was 0.81 and 0.85,the Kendall harmony coefficient was 0.162~0.258,and the variation coefficient of three-level items was 0.00~0.190.The ERAS rehabilitation nursing practice protocol included 4 first-level indicators,25 second-level indi-cators and 76 third-level indicators.Conclusion The perioperative ERAS rehabilitation nursing practice protocol which integrated traditional Chinese and western medicine for gynecological tumor patients covered the perioperative nursing strategies of traditional Chinese and western medicine,which was scientific,comprehensive and practical,and could provide reference for promoting the development of rehabilitation nurs-ing for gynecological tumor patients.
