摘　　要：目的:探讨术中腔内超声引导下根治高位多间隙肛周脓肿的安全性及可行性。方法:选取2021年8月至2022年12月于成都中医药大学附属医院肛肠科住院且拟行根治性手术治疗的高位多间隙肛周脓肿患者86例,随机分为观察组和对照组,各43例,对照组经验指导下行根治术,观察组术中直肠腔内超声引导下行根治术。比较术中发现内口率、手术切口数、术后不同时间疼痛程度、住院时间,以及治愈率。结果:观察组术中精准发现内口率(90.70%)明显高于对照组(81.40%),切口2~3个切口者占比(65.12%)明显高于对照组(37.21%),术后第1、3、5、7天疼痛VAS评分明显低于对照组,住院时间明显短于对照组,治愈率(95.35%)明显高于对照组(88.37%),P<0.05。结论:术中腔内超声引导下行根治性手术治疗高位多间隙肛周脓肿,可准确发现内口,为合理设计切口提供依据,提高治愈率,而且可减轻患者术后疼痛,缩短住院时间。Objective To investigate the safety and feasibility of intraoperative intracavity ultrasound-guided radical treatment of high multi-space perianal abscess.MethodsA total of 86 patients with high multi-space perianal abscess who were admitted to the anorectal department of Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from August 2021 to December 2022 and planned to undergo radical surgery were randomly divided into the observation group and the control group,43 cases in each group.The control group was guided by experience to undergo radical surgery,while the observa-tion group was guided by intrarectal ultrasound to undergo radical surgery.The rate of intraoperative inter-nal openings,the number of surgical incisions,the degree of pain at different time after surgery,the length of hospital stay,and the cure rate were compared.Results The intraoperative accurate detection rate of the internal openings(90.70%)in the observation group was significantly higher than that in the control group(81.40%).The proportion of 2 to 3 incisions(65.12%)was significantly higher than that in the control group(37.21%).The pain VAS scores at the lst,3rd,5th and 7th days after surgery were significantly lower than those in the control group,and the length of hospitalization was significantly shor-ter than that in the control group.The cure rate(95.35%)was significantly higher than that of the con-trol group(88.37%)(P<0.05).Conclusion Intraoperative intracavity ultrasound-guided radical treatment of high multi-space perianal abscess can accurately detect the internal openings,provide basis for rational incision design,improve the cure rate,reduce postoperative pain and shorten hospital stay.
