不同体位超声检查对先天性肛门直肠畸形的诊断价值

摘　　要：目的:探讨不同体位超声检查对先天性肛门直肠畸形(CARM)的诊断价值。方法:选择2020年4月至2022年4月我院收治的100例CARM患儿为研究对象,患儿术前均行抱立位、仰卧位超声检查和倒立位X线检查,以手术结果为金标准,比较抱立位、仰卧位超声及倒立位X线检查对CARM的诊断效果。结果:手术结果证实低位44例,中高位56例。抱立位超声诊断:低位42例,中高位58例,其诊断准确率、灵敏度、特异性及Kappa值分别为96.00%、93.18%、98.21%、0.918;仰卧位超声诊断:低位41例,中高位59例,其诊断准确率、灵敏度、特异度及Kappa值分别为95.00%、90.90%、98.21%、0.898;倒立位X线诊断:低位43例,中高位57例,其诊断准确率、灵敏度、特异度及Kappa值分别为83.00%、79.55%、85.71%、0.654。抱立位、仰卧位超声检查的诊断准确率均高于倒立位X线检查(P<0.05),且抱立位、仰卧位超声检查与手术结果的一致性Kappa值均高于倒立位X线检查(P<0.05)。结论:抱立位、仰卧位超声检查均对CARM具有良好的诊断效能。Objective To evaluate the diagnostic value of ultrasonography in different positions in con-genital anorectal malformation(CARM).Methods A total of 1o0 children with CARM admitted to our hospital from April 2020 to April 2022 were selected as the research objects.All the children underwent preoperative ultrasound examination in orthostatic and supine positions,and received X-ray examination in inverted position.The diagnostic effect of ultrasound examination in orthostatic and supine positions and X-ray examination in inverted position was compared,with surgical results as the gold standard.Results The results of operation confirmed:There were 44 cases of low typed and 56 cases of intermediate and high typed CARM.Ultrasound examination in orthostatic position:There were 42 cases of low typed and 58 cases of intermediate and high typed CARM,and the diagnostic accuracy,sensitivity,specificity and Kappa value were 96.00%,93.18%,98.21%and 0.918,respectively.Ultrasound examination in supine position:There were 4l cases of low typed and 59 cases of intermediate and high typed CARM,and the diagnostic accuracy,sensitivity,specificity and Kappa value were 95.00%,90.90%,98.21% and 0.898,respectively.X-ray diagnosis in inverted position:There were 43 cases of low typed and 57 cases of intermediate and high typed CARM.The diagnostic accuracy,sensitivity,specificity and Kappa value were 83.00%,79.55%,85.71%and 0.654,respectively.The diagnostic accuracy of ultrasound examination in orthostatic and supine positions was higher than that of X-ray examination in inverted position(P<0.05).The Kappa value of ultrasound examination in orthostatic and supine positions indicating the consistency with the surgical results was higher than that of X-ray examination in inverted position(P<o.05).Conclusion Ultrasound examination in orthostatic and supine positions has good diagnostic efficacy for CARM.
