摘  要:探讨雅培i2000化学发光微粒子免疫分析法(CMIA)筛查梅毒螺旋体特异性抗体的测量阈值,并对CMIA检测出的假阳性样本进行分析。对北京市通州区中西医结合医院2015年1月—2019年1月间100例年龄为20~42岁孕妇梅毒样本测定值S/CO为1~3.5的样本高速离心后,再进行CMIA法检测梅毒螺旋体特异性抗体,对这些阳性样本进行密螺旋体颗粒凝集试验(TPPA)确证实验,用酶免ELISA测定,用梅毒快速血浆反应素试验(RPR)测定。用其他的检测方法作为对照组。结果显示,100例样本中CMIA法筛查出98例测定值S/CO为1~3.5与初次测定值一样。其中两例为离心不彻底导致假阳性。98例阳性样本进行TPPA确证,结果为阴性。用酶免ELISA测定98例阳性标本,均为阴性。用梅毒快速血浆反应素试验(RPR)测定98例阳性样本,均为阴性。测定组和对照组检测结果差异具有统计学意义(P<0.05)。研究发现,CMIA法检测梅毒特异性抗体的敏感性高,可作为筛查实验。CMIA法检测梅毒特异性抗体的S/CO值在1~3.5之间的样本需高速离心后再进行TPPA确证,酶免ELISA和RPR的测定。Exploring the measurement threshold of Abbott i2000 chemiluminescent particle immunoassay(CMIA)for screening specific antibodies against Treponema pallidum,and analyzing false positive samples detected by CMIA.From January 2015 to January 2019,100 samples of syphilis samples from pregnant women aged 20-42 years old at the Tongzhou District Integrated Traditional Chinese and Western Medicine Hospital were centrifuged at high speed with a S/CO value of 1-3.5.Then,CMIA method was used to detect Treponema pallidum specific antibodies.These positive samples were confirmed by Treponema pallidum particle agglutination assay(TPPA),enzyme-linked immunosorbent assay(ELISA),and rapid plasma reaction(RPR)for syphilis.Use other testing methods as the control group.The results showed that 98 out of 100 samples were screened using the CMIA method,and the measured values of S/CO ranged from 1 to 3.5,which were the same as the initial measurement values.Two of them were false positives due to incomplete centrifugation.98 positive samples were confirmed by TPPA,and the result was negative.98 positive specimens were tested using enzyme-linked immunosorbent assay(ELISA),all of which were negative.98 positive samples were tested using the rapid plasma reaponin test(RPR)for syphilis,all of which were negative.The difference in test results between the measurement group and the control group was statistically significant(P<0.05).Research has found that the CMIA method has high sensitivity in detecting syphilis specific antibodies and can be used as a screening experiment.Samples with S/CO values between 1-3.5 for detecting syphilis specific antibodies using the CMIA method require high-speed centrifugation followed by TPPA confirmation,enzyme-linked immunosorbent assay(ELISA),and RPR determination.
