《高血压冠心病单方验方大全》治疗冠心病、心绞痛、心肌梗塞的用药规律研究

摘　　要：目的探讨名医论治冠心病的用药规律,为临床用药提供借鉴。方法整理《高血压冠心病单方验方大全》中名医论治冠心病的有效经验方,将复方录入Excel 2010建立数据库,采用古今医案云平台(V2.3)进行单味中药的频次分析、中药属性频次分析、关联规则以及聚类分析,采用SPSS Statistics 25.0进行因子分析。结果共纳入554首经验方,冠心病通用方以丹参、川芎、党参、黄芪、甘草、当归、赤芍、茯苓、瓜蒌、桂枝、红花、郁金、麦冬、五味子、薤白为高频药物。除此之外,心绞痛方和心肌梗塞方高频药物还包含半夏、三七、延胡索。三类方药性均以温为主,其次为平、微寒;药味均以甘、苦、辛为主。归经均以心经为核心,与肝、肺、脾经密切相关。功效以活血化瘀为主,辅以通经止痛。三类方关联规则各获得8个核心药对,如川芎-丹参、瓜蒌-薤白、黄芪-党参等。因子分析从冠心病方和心肌梗塞方药物中各提取了6个因子,从心绞痛方药物中提取了5个因子。聚类分析将三类方药物各分为4个组合,共奏活血化瘀、益气养阴、燥湿祛痰之效。结论该项数据挖掘共获得生脉散、瓜蒌薤白半夏汤、枳实薤白桂枝汤、活血养心组合等方剂,可将冠心病方、心绞痛方、心肌梗塞方的治法概括为祛痰化瘀,益气养阴。Objective To explore the medication rules of famous doctors in the treatment of coronary heart disease,and provide reference for clinical medication.Methods The effective experience prescription of famous doctors in Gaoxueya Guanxinbing Dan-fang Yanfang Daquan for treating coronary heart disease was collected,compound was input into Excel 2010 to establish data-base,the ancient and modern medical case cloud platform(V2.3)was used for frequency analysis of single Chinese medicine,fre-quency analysis of Chinese medicine attributes,association rules and cluster analysis,and SPSS Statistics 25.0 was used for factor analysis.Results A total of 554 empirical prescriptions were included.The general prescriptions for coronary heart disease took Danshen(Salviae Miltiorrhizae Radix Et Rhizoma),Chuanxiong(Chuanxiong Rhizoma),Dangshen(Codonopsis Radix),Huangqi(Astragali Radix),Gancao(Glycyrrhizae Radix Et Rhizoma),Danggui(Angelicae Sinensis Radix),Chishao(Paeoniae Radix Rubra),Fuling(Poria),Gualou(Trichosanthis Fructus),Guizhi(Cinnamomi Ramulus),Honghua(Carthami Flos),Yujin(Cur-cumae Radix),Maidonng(Ophiopogonis Radix),Wuweizi(Schisandrae Chinensis Fructus)and Xiebai(Allii Macrostemonis Bul-bus).In addition,the high-frequency drugs of angina pectoris prescription and myocardial infarction prescription also included Banxia(Pinelliae Rhizoma),Sanqi(Notoginseng Radix Et Rhizoma)and Yanhusuo(Corydalis Rhizoma).The drugs of the three kinds of prescriptions were mainly warm,followed by mild and mild cold.The medicine was mainly sweet,bitter and pungent.The heart meridian was the core of meridians,which was closely related to liver,lung and spleen meridians.The efficacy was mainly to promote blood circulation and remove blood stasis,supplemented by dredging menstruation and relieving pain.The association rules of the three types of parties obtain 8 core drug pairs,such as Chuanxiong(Chuanxiong Rhizoma)-Danshen(Salviae Miltior-rhizae Radix Et Rhizoma),Gualou(Trichosanthis Fructus)-Xiebai(Allii Macrostemonis Bulbus)and Huangqi(Astrag
