基于“诸痛痒疮,皆属于心”浅谈放射性皮炎

摘　　要：放射性皮炎是放射治疗后常见的皮肤粘膜损伤性疾病,临床上暂无法完全避免,一旦发生对患者的生活和工作会产生不便,甚至影响放疗的进程。西医针对该病尚无有效的标准治疗方案。近年来,中医在放射性皮炎方面发挥了一定疗效,同时具有简便廉效快的特点。该文基于“诸痛痒疮,皆属于心”的中医理论,从心属火主痛,心主血脉主血证,心藏神主痒三方面对该病进行病因病机的分析,总结出在治疗上应根据患者的具体病情,依据清热不忘养阴、活血兼顾养血、调神以止痒痛的原则去进行辨证论治,以减轻患者的皮肤损伤,改善其生活质量,以飨同道,服务临床。Radiation dermatitis is a common skin side effect after radiotherapy,which cannot be completely avoided.Once it occurs,it will cause inconvenience to the patient's life and work and affect the process of radiotherapy.There is no effective standard treatment plan for this disease in western medicine.In recent years,traditional Chinese medicine has exerted a certain curative effect in radiation dermatitis and has the characteristics of simplicity,cheapness and quickness.This article is based on the traditional Chinese medicine theory of“all pain and itching ulcers are related to heart”.It analyzes the etiology and pathogen-esis of this disease from three aspects:the heart belongs to fire of five elements and dominates pain,the heart belongs to blood ves-sels and blood syndromes,and the heart stores the spirit and controls itching.It is concluded that the treatment should be based on the specific condition of the patients,and based on the principles of clearing heat without forgetting to nourish Yin,promoting blood circulation while nourishing blood,and regulating the mind to relieve itching and pain,in order to reduce skin damage,im-prove their quality of life and serve the clinical practice.
