清火炎消汤联合牙周治疗与义齿修复对牙列缺损伴慢性牙周炎患者龈沟液炎症指标、牙齿功能影响

摘　　要：目的观察清火炎消汤联合牙周治疗与义齿修复对牙列缺损伴慢性牙周炎患者龈沟液炎症指标、牙齿功能影响。方法选取自2019年2月—2022年2月该院口腔医学中心收治的80例牙列缺损伴慢性牙周炎患者作为研究对象。根据随机原则分为对照组和观察组各40例。对照组常规给予牙周基础治疗与义齿修复治疗,观察组在对照组基础上给予清火炎消汤治疗。比较两组牙周健康、龈沟液炎症相关指标及咀嚼功能、吞咽功能、中医证候积分的差异,统计两组疗效和不良反应情况。结果治疗前,两组牙周健康相关指标、咀嚼功能、吞咽功能及中医证候积分比较,差异无统计学意义(P>0.05),治疗后经统计学检验发现,两组牙周探诊深度、牙齿松动度、出血指数及中医证候积分均较前下降,咀嚼功能、吞咽功能较前差异无统计学意义(P>0.05),观察组治疗后牙周健康相关指标、中医证候积分下降后的数值低于对照组,咀嚼功能、吞咽功能较对照组差异无统计学意义(P>0.05)。治疗前,两组龈沟液炎症相关指标比较,差异无统计学意义(P>0.05),治疗后经统计学检验发现,两组龈沟液高敏-C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、趋化因子CX3CL1均较前下降,且观察组龈沟液炎症相关指标下降后的数值均低于对照组,差异有统计学意义(P<0.05)。观察组总有效率为92.50%(37/40),明显高于对照组的75.00%(30/40),差异有统计学意义(P<0.05)。结论清火炎消汤联合牙周治疗与义齿修复治疗牙列缺损伴慢性牙周炎可改善牙周状况,减轻炎症反应,提高疗效。Objective To observe the effect of Qinghuo Yanxiao Decoction(清火炎消汤)combined with periodontal therapy and denture restoration on gingival crevicular fluid inflammation indexes and tooth function in patients with dentition defect and chronic periodontitis.Methods From February 2019 to February 2022,80 patients with dentition defect and chronic periodontitis who were admitted to the Stomatological Center of the hospital were selected as the research subjects.According to the principle of randomization,they were divided into control group and observation group with 40 cases in each group.The control group was giv-en regular periodontal basic treatment and denture restoration treatment,and the observation group was given Qinghuo Yanxiao De-coction on the basis of the control group.The differences in periodontal health,gingival crevicular fluid inflammation-related in-dicators,masticatory function,swallowing function and TCM syndrome scores were compared between the two groups,and the cura-tive effects and adverse reactions of the two groups were calculated.Results Before treatment,there were no significant differences in periodontal health-related indicators,chewing function,swallowing function or TCM syndrome scores between the two groups(P>0.05).Mobility,bleeding index and TCM syndrome scores were all decreased compared with those before treatment,but there was no statistical difference in masticatory function or swallowing function(P>0.05).Compared with those of the control group,there was no statistical difference in chewing function or swallowing function(P>0.05).Before treatment,there was no statistical difference between the two groups of gingival crevicular fluid inflammation-related indicators(P>0.05).After treat-ment,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and chemokine CX3CL1 were all decreased and the decreased values of gingival crevicular fluid inflammation-related indexes in the observation group were all lower than those in the control group(P<0.05).The total effecti
