自拟散寒化瘀通络方对湿寒痹阻型类风湿性关节治疗效果观察  

Effect of Self-made Sanhan Huayu Tongluo Decoction(自拟散寒化瘀通络方)on Rheumatoid Arthritis of Damp-Cold Obstruction Type

摘　　要：目的观察自拟散寒化瘀通络方对湿寒痹阻型类风湿性关节炎(rheumatoid arthritis,RA)的临床疗效。方法按随机数字表法将该院2020年1月—2022年1月收治的98例湿寒痹阻型RA患者分为观察组和对照组两组,每组49例。对照组给予戈利木单抗治疗,观察组在对照组基础上联合自拟散寒化瘀通络方治疗。治疗2个月后,比较两组患者临床疗效、不良反应发生率,同时比较治疗前后两组患者关节功能恢复情况、实验室相关指标、炎症因子水平、细胞免疫功能指标。结果治疗后,观察组患者治疗总有效率为95.92%(47/49)明显高于对照组的79.59%(39/49)(P<0.05);观察组患者晨僵时间、关节疼痛指数、关节肿胀数、关节压痛数均明显低于对照组,差异具有统计学意义(P<0.05);观察组患者血沉(ESR)、类风湿因子(RF)、C反应蛋白(CRP)水平均明显低于对照组,差异具有统计学意义(P<0.05);观察组患者白细胞介素-6(IL-6)、血清白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平均明显低于对照组(P<0.05);观察组患者CD_(3)^(+)、CD_(4)^(+)水平明显高于对照组,CD_(8)^(+)水平明显低于对照组,差异具有统计学意义(P<0.05);两组患者不良反应总发生率比较差异均无统计学意义(P>0.05)。结论自拟散寒化瘀通络方联合戈利木单抗对湿寒痹阻型RA患者具有一定治疗效果,能改善患者相关临床症状,促进患者关节功能恢复,不良反应较少,具有一定安全性。Objective To observe the clinical effect of Self-made Sanhan Huayu Tongluo Decoction(自拟散寒化瘀通络方)in the treatment of rheumatoid arthritis(RA)of damp-cold obstruction type.Methods Ninety-eight patients with RA of damp-cold obstruction type were admitted to the hospital from January 2020 to January 2022.They were divided into observation group and control group by random number table method,with 49 cases in each group.The patients in the control group were treated with golimumab,and those in the observation group were treated with Self-made Sanhan Huayu Tongluo Decoction combined with golimumab.After 2 months of treatment,the clinical efficacy and the incidence of adverse reactions were compared between the two groups.Meanwhile,joint function recovery,relevant laboratory indicators,inflammatory factors and cellular immune function indicators were compared between the two groups before and after treatment.Results The total treatment response rate in the ob-servation group(95.92%,47/49)was significantly higher than that in the control group(79.59%,39/49)(P<0.05).The du-ration of morning stiffness,joint pain index,swollen joint count and tender joint count in the observation group were significantly shorter/lower than those in the control group(P<0.05).The levels of erythrocyte sedimentation rate(ESR),rheumatoid factor(RF),C-reactive protein(CRP),interleukin-6(IL-6),serum interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)in the observation group were significantly lower than those in the control group(P<0.05).The levels of CD_(3)^(+) and CD_(4)^(+) in the observation group were significantly higher than those in the control group,and the level of CD_(8)^(+) was significantly low-er than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Self-made Sanhan Huayu Tongluo Decoction combined with golimumab is effective in the treatment of patients with RA of damp-cold obstruction type,and can imp
