五加生化胶囊治疗产后出血的疗效及对产妇凝血功能、炎症反应的影响

摘　　要：目的分析五加生化胶囊治疗产后出血的疗效及对产妇凝血功能、炎症反应的影响。方法2019年9月—2021年8月河北北方学院附属第二医院产科收治的产后出血产妇100例,通过随机数字表法分为研究组50例及对照组50例。两组均接受常规治疗,对照组接受米索前列醇片治疗,研究组接受五加生化胶囊联合米索前列醇片治疗,两组均治疗7 d。比较两组出血情况,治疗前、治疗后两组子宫下降高度及凝血功能、炎症反应,治疗后疗效。结果研究组用药后24 h出血量低于对照组,出血持续时间、恶露持续时间短于对照组(P<0.05)。治疗后,两组血浆纤维蛋白原(FIB)和D-二聚体(D-D),血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)和白细胞介素-8(IL-8)水平较治疗前均降低,且研究组低于对照组(P<0.05);治疗后,两组子宫下降高度均较治疗前增加,研究组高于对照组(P<0.05)。治疗后,研究组总有效率高于对照组[94.00%(47/50)vs 80.00%(40/50),P<0.05]。结论五加生化胶囊治疗产后出血可提高出血控制效果,促进子宫复原,或许与其可调节机体凝血功能、抑制炎症反应有关,值得临床进一步研究与应用。Objective To analyze the effect of Wujia Shenghua Capsule(五加生化胶囊)in the treatment of postpartum hem-orrhage and its effect on maternal blood coagulation function and inflammatory response.Methods A total of 100 patients with postpartum hemorrhage admitted to the obstetrics department of The Second Affiliated Hospital of Hebei North University from September 2019 to August 2021 were divided into the study group(50 cases)and the control group(50 cases)by random number table method.Both groups received conventional treatment.The control group received misoprostol tablets and the study group re-ceived Wujia Shenghua Capsule combined with misoprostol tablets.Both groups received 7 d of treatment.The bleeding,descend-ing height of the uterus,coagulation function and inflammatory response before treatment and after treatment,and the curative effect after treatment of the two groups were compared.Results The 24 h bleeding volume after medication in the study group was lower in the two groups,the duration of bleeding and duration of lochia in the study group were shorter in the two groups(P<0.05).The levels of plasma fibrinogen(FIB)and D-dimer(D-D),serum C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8)in the two groups after treatment were lower than those before treatment,and the levels in the study group were lower(P<0.05).The descending height of the uterus in both groups after treatment was higher than that before treatment,and the descending height of the study group was higher than that in the two groups(P<0.05).After treatment,the total effective rate of the study group was higher than that of the control group[94.00%(47/50)vs 80.00%(40/50),P<0.05].Conclusion Wujia Shenghua Capsule in the treatment of postpartum hemorrhage can improve the effect of bleeding control and promote the recovery of the uterus,which may be related to its ability to regulate the coagulation function and inhibit the in-flammatory response,worthy of further clinical research and application.
