穴位埋线联合健脾祛湿中药对肥胖并发高脂血症脂质代谢及内脏脂肪的影响

摘　　要：目的探讨穴位埋线联合健脾祛痰中药对肥胖并发高脂血症脂质代谢内脏脂肪的影响及可能机制。方法选取40例肥胖并发高脂血症患者随机分为两组,针药组20例(穴位埋线每月1次联合加减参苓白术散日1剂,2次/d,早晚服用),中药组20例(加减参苓白术散服用方法同上),两组疗程均为3个月,观察两组治疗前后肥胖指标:体质量、体质指数(BMI)、腰臀比、体脂百分率(F%),肥胖度(A%);血脂指标:总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C);内脏脂肪指标:内脏脂肪指数(VAI)变化。结果针药组和中药组在治疗肥胖总有效率均能达到85%以上,且针药组略高于中药组。与治疗前相比,针药组、中药组均能降低肥胖指标和内脏脂肪指标,并能改善血脂指标(P<0.05)。与中药组相比,针药组治疗后在降低肥胖指标、内脏脂肪指标及改善血脂指标方面效果更加显著(P<0.05)。结论针药组与中药组治疗均有减肥降脂作用,针药联合使用具有协同作用,临床效果更明显,穴位埋线联合健脾祛湿中药法可能通过调节“脂肪-胰岛素轴”,改善胰岛素抵抗,降低胆固醇的合成,从而纠正患者脂质紊乱,使患者达到更好的减肥降脂双重功效,进一步降低患者肥胖相关疾病及代谢综合征的风险,可在临床上推广应用。Objective To investigate the effect of acupoint catgut embedding combined with Chinese medicine for strengthening spleen and removing dampness on visceral fat in obesity complicated with hyperlipidemia and its possible mechanism.Methods Forty obese patients with hyperlipidemia were randomly divided into 2 groups,20 in the acupuncture group[catgut embedding at acupoints once a month combined with Modified Shenling Baizhu Powder(加减参苓白术散)1 dose a day,twice a day,taking in the morning and evening].Twenty patients in the traditional Chinese medicine group(with Modified Shenling Baizhu Powder tak-ing the same method as the above).The treatment course of the two groups was 3 months,and the obesity indicators of the two groups before and after treatment were observed:body mass,body mass index(BMI),waist-to-hip ratio,body fat percentage(F%),obesity(A%);blood lipid index:total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C);visceral fat index:changes in visceral fat index(VAI).Results The total efficiency of the acu-puncture group and the traditional Chinese medicine group in the treatment of obesity can reach more than 85%,and that of the acupuncture group was slightly higher than that of the traditional Chinese medicine group.Compared with those before treatment,the acupuncture group and the Chinese medicine group can reduce the obesity index and the visceral fat index,and can improve the blood lipid index(P<0.05).Compared with the traditional Chinese medicine group,the acupuncture group had more signifi-cant effects in reducing obesity indexes,visceral fat indexes and improving blood lipid indexes after treatment(P<0.05).Con-clusion Both the acupuncture group and the traditional Chinese medicine group have the effect of weight loss and lipid lowering.The combined use of acupuncture and medicine has a synergistic effect,and the clinical effect is more obvious.Acupoint catgut embedding combined with Chinese medicine for strengthening spleen and removing dampnes
