方志军从“肝”论治结直肠癌肝转移经验

摘　　要：结直肠癌是临床最多见的消化系统恶性肿瘤,具有发病率高、病死率高的特点,其中肝转移是危害结直肠癌患者生存期的最关键因素。干预转移是结直肠癌诊治的紧要举措。方志军教授根据古今医家认识及多年临床经验提出从“肝”论述结直肠癌肝转移,诊治全程需重肝,若肝风内动,当平肝熄风,潜阳降逆;若肝血瘀阻,宜养肝化瘀,活血与止血并行;若肝气失调,需身心同治,疏肝理气。同时,注重因人制宜、中西结合治之。如此,气血调和,阴阳平衡,则邪祛而正安。Colorectal cancer is the most common clinical malignant tumor of the digestive system,with high morbidity and mortality,among which liver metastasis is the most critical factor affecting the survival of colorectal cancer patients.Metastasis intervention is an important measure in the diagnosis and treatment of colorectal cancer.Professor FANG Zhijun,based on the understanding of ancient and modern doctors and years of clinical experience,proposed to discuss liver metastasis of colorectal cancer from“liver”.The whole process of diagnosis and treatment pays attention to liver.If the liver wind moves inward,it should calmliver and extinguish wind,subdue Yang and lower the adverse reactions.If there is blood stasis in liver,it is advisable to nourish liver and dissipate blood stasis,promote blood circulation and stop bleeding in parallel.If liver Qi imbalance occurs,it is necessary to treat it both physically and mentally,and regulate liver and Qi.At the same time,the attention should be paid to adapting measures to individual needs and combining traditional Chinese and Western treatment.In this way,the treatment can make the harmony of Qi and blood,the balance of Yin and Yang and the elimination of evils.
