滋阴化瘀法在妊娠晚期羊水偏少患者中的应用

摘　　要：目的探究滋阴化瘀法在妊娠晚期羊水偏少患者中的应用效果。方法选取2020年9月—2022年5月该院产科收治的妊娠晚期肾虚血瘀型羊水偏少患者90例,采用简单随机法分为对照组45例,观察组45例。对照组给予常规方案治疗,观察组在对照组常规方案治疗的基础上给予滋阴化瘀中药治疗,两组患者均治疗4周。比较两组患者治疗前、治疗1周、4周后患者羊水指数(amniotic fluid index,AFI),比较两组患者围产期羊水情况[羊水Ⅱ~Ⅲ污染率、产时羊水量]、围生儿情况[胎儿窘迫率、新生儿评分(Apgar)、新生儿窒息率],并记录不良反应事件发生情况。结果两组患者完成4周治疗后,观察组羊水Ⅱ~Ⅲ级污染率、胎儿窘迫率、新生儿窒息率明显低于对照组(P<0.05);两组患者完成4周治疗后,观察组AFI值、产时羊水量、新生儿Apgar评分明显高于对照组(P<0.05);两组患者治疗期间未诉明显不良反应。结论滋阴化瘀法结合常规方案可明显提高妊娠晚期羊水偏少患者AFI值,改善患者围产期羊水情况,降低胎儿窘迫率和新生儿窒息率,提高新生儿Apgar评分,值得推广应用。Objective To explore the application effects of nourishing Yin and resolving blood stasis method on patients with oligohydramnios in late pregnancy.Methods A total of 90 patients with oligohydramnios due to kidney deficiency and blood stasis in late pregnancy who were treated in department of obstetrics were enrolled between September 2020 and May 2022,and were divided into the control group(45 cases)and the observation group(45 cases)by simple random method.The control group was given the conventional treatment while the observation group was treated with Chinese drug agentia for nourishing Yin and removing blood stasis on the basis of the conventional treatment in the control group.Both groups were treated for 4 weeks.The amniotic fluid index(AFI)before treatment and after 1 and 4 weeks of treatment and perinatal amniotic fluid status[amniotic fluid gradeⅡ-Ⅲcontamination rate,intrapartum amniotic fluid volume]and perinatal conditions[fetal distress rate,neonatal score(Apgar),neonatal asphyxia rate]were compared between the two groups,and the occurrence of adverse events were recorded.Results The amniotic fluid gradeⅡ-Ⅲcontamination rate,fetal distress rate and neonatal asphyxia rate in the observation group after 4 weeks of treatment were significantly lower than those in the control group(P<0.05).After 4 weeks of treatment,the AFI value,intrapartum amniotic fluid volume and neonatal Apgar score were significantly higher in the observation group compared with those in the control group(P<0.05).No obvious adverse reactions occurred in both groups during treatment.Conclusion Nourishing Yin and resolving blood stasis method combined with conventional regimen can significantly enhance the AFI value of patients with oligohydramnios in late pregnancy,improve the perinatal amniotic fluid condition,reduce the fetal distress rate and neonatal asphyxia rate and increase the Apgar score of neonates.
