血府逐瘀口服液治疗慢性阻塞性肺疾病急性加重期合并肺动脉高压的临床研究

摘　　要：目的探讨血府逐瘀口服液治疗慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)合并肺动脉高压(pulmonary arterial hypertension,PAH)的临床疗效。方法回顾性收集2021年5月—2022年5月广元市中医医院收治的AECOPD合并PAH患者100例的临床资料,根据治疗方法不同分为对照组(63例)和观察组(37例)。对照组给予常规治疗和法舒地尔治疗,观察组加用血府逐瘀口服液治疗。比较两组治疗总有效率,治疗前后肺功能、动脉血气、肺动脉压、凝血指标、炎性因子变化以及不良反应。结果持续治疗2周后,观察组治疗总有效率91.89%(32/37),显著高于对照组的74.60%(47/63)(P<0.05)。治疗后,观察组用力呼气肺活量(forced expiratory vital capacity,FVC)、1秒用力呼气容积(1 second forced expiratory volume,FEV1)、氧分压(partial pressure of oxygen,PaO_(2))显著高于对照组,二氧化碳分压(partial pressure of carbon dioxide,PaCO_(2))和平均动脉压(mean arterial pressure,mPAP)显著低于对照组,纤维蛋白原(fibrinogen,FIB)、D-二聚体(D-Dimer,D-D)及抗凝血酶Ⅲ(antithrombinⅢ,ATⅢ)显著低于对照组,以及血浆白细胞介素-1(interleukin-1,IL-1)、白细胞介素-6(interleukin-6,IL-6)显著低于对照组,差异均有统计学意义(P<0.05)。治疗期间,观察组、对照组不良反应率分别为21.62%(8/37)和17.46%(11/63),差异无统计学意义(P>0.05)。结论血府逐瘀口服液可有效提高AECOPD合并PAH患者肺功能、改善动脉血气、降低肺动脉压,同时改善机体高凝状态,减轻机体炎症反应,疗效确切,安全可靠。Objective To explore the clinical efficacy of Xuefu Zhuyu Oral Liquid(血府逐瘀口服液)in the adjuvant treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with pulmonary arterial hypertension(PAH)and its effects on pulmonary function,blood coagulation indexes and inflammatory factors.Methods The clinical data of 100 patients with AECOPD complicated with PAH admitted to Guangyuan Hospital of Traditional Chinese Medicine from May 2021 to May 2022 was retrospectively collected and divided into control group(63 cases)and observation group(37 cases)according to different treatment methods.The control group was given routine treatment and fasudil,and the observation group was additionally given Xuefu Zhuyu Oral Liquid.The total effective rate of treatment,and pulmonary function,arterial blood gas,pulmonary artery pressure,blood coagulation indexes,inflammatory factors and adverse reactions before and after treatment were compared between the two groups.Results After 2 weeks of continuous treatment,the total effective rate of treatment in the observation group was significantly higher than that in the control group[91.89%(32/37)vs 74.60%(47/63)](P<0.05).After treatment,the levels of forced expiratory volume(FVC),1 second forced expiratory volume(FEV1)and oxygen partial pressure(PaO_(2))were significantly higher in the observation group than those in the control group while the levels of partial pressure of carbon dioxide(PaCO_(2))and mean arterial pressure(mPAP)were significantly lower than those in the control group,and the levels of fibrinogen(FIB),D-dimer(DD)and antithrombin III(ATⅢ)were significantly lower than those in the control group(P<0.05),and the levels of plasma interleukin-1(IL-1)and interleukin-6(IL-6)were significantly lower than those in the control group(P<0.05).During the treatment period,the adverse reaction rate was 21.62%(8/37)in the observation group and 17.46%(11/63)in the control group(P>0.05).Conclusion Xuefu Zhuyu Oral Liquid can effectively improve the pulmon
