加味消痈解毒汤联合青霉素和甲硝唑治疗肾脓肿患者的疗效及对血生化和肾功能的影响

摘　　要：目的研究加味消痈解毒汤联合青霉素和甲硝唑治疗肾脓肿患者的疗效及对血生化和肾功能影响。方法选取2019年10月—2021年10于唐山市中医医院接受治疗的82例肾脓肿患者为研究对象,按照治疗方法分为加味消痈组(42例)及常规治疗组(40例),常规治疗组给予青霉素和甲硝唑治疗,加味消痈组在此基础上给予加味消痈解毒汤治疗,比较两组患者的临床疗效、中医证候积分、血生化指标[血清β2微球蛋白(serumβ2 microglobulin,sβ2-MG)、胱抑素C(Cystatin C,CysC)、视黄醇结合蛋白(retinol binding protein,RBP)]及肾功能[肌酐(serum creatinine,Scr)、尿素氮(blood urea nitrogen,BUN)、尿酸(uric acid,UA)]水平、不良反应发生率。结果加味消痈组临床有效率85.71%(36/42)高于常规治疗组61.67%(26/40)(P<0.05);治疗后,两组中医证候积分均有所降低,且加味消痈组均低于常规治疗组(P<0.05);治疗后,两组sβ2-Mg,CysC、sRBP水平均有所降低,且加味消痈组水平低于常规治疗组(P<0.05);治疗后两组Scr、BUN、UA均有所降低,且加味消痈组低于常规治疗组(P<0.05)。治疗后,两组不良反应发生率差异无统计学意义(P>0.05)。结论加味消痈解毒汤干预可以改善患者临床治愈率,改善证候疗效和实验室指标,用于临床造福肾脓肿患者。Objective To study the curative effect of Modified Xiayong Jiedu Decoction(加味消痈解毒汤)combined with penicillin and metronidazole on patients with renal abscess and its influences on blood biochemistry and renal function.Methods A total of 82 patients with renal abscess treated in the hospital were enrolled as the research objects between October 2019 and October 2021.According to different treatment methods,they were divided into Modified Xiayong Jiedu Decoction group(42 cases)and routine treatment group(40 cases).The routine treatment group was given penicillin and metronidazole,while Modified Xiayong Jiedu Decoction group was additionally given Modified Xiayong Jiedu Decoction.The clinical curative effect,the scores of TCM symptoms,the levels of blood biochemistry indexes[serumβ2 microglobulin(sβ2-MG),cystatin C(CysC),retinol binding protein(RBP)]and renal function indexes[creatinine(Scr),blood urea nitrogen(BUN),uric acid(UA)]and the incidence of adverse reactions were compared between the two groups.Results The clinical response rate of Modified Xiayong Jiedu Decoction group was higher than that of the routine treatment group[85.71%(36/42)vs 61.67%(26/40),P<0.05].After treatment,the scores of TCM syndromes in both groups were decreased,which were lower in the Modified Xiayong Jiedu Decoction group than those in the routine treatment group(P<0.05).After treatment,the levels of sβ2-MG,CysC and sRBP in both groups were decreased,which were lower in the Modified Xiayong Jiedu Decoction group than those in the routine treatment group(P<0.05).After treatment,the levels of Scr,BUN and UA in both groups were decreased,which were lower in the Modified Xiayong Jiedu Decoction group than those in the routine treatment group(P<0.05).After treatment,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Modified Xiayong Jiedu Decoction can increase the clinical cure rate,improve the curative effect on symptoms and laboratory indexes,with clinical b
