摘　　要：目的探讨超声引导下肌筋膜触发点针刺联合靶点射频消融术治疗腰背部疼痛患者的临床效果。方法选取2021年1月至2022年2月收治的80例腰背部疼痛患者,将其随机分为对照组和观察组,各40例。对照组采用单纯靶点射频消融术治疗,观察组采用超声引导下肌筋膜触发点针刺联合靶点射频消融术治疗。比较两组的治疗效果。结果观察组的治疗总有效率显著高于对照组(P<0.05)。治疗后,观察组的压力疼痛感觉阈(PPDT)和压力疼痛耐受阈(PPTO)高于对照组(P<0.05)。治疗后,观察组的疼痛分级指数(PRI)、现时疼痛强度(PPI)、Oswestry功能障碍指数(ODI)评分低于对照组(P<0.05)。治疗后,观察组的皮质醇(Cor)、促肾上腺皮质激素(ACTH)水平低于对照组(P<0.05)。结论超声引导下肌筋膜触发点针刺联合靶点射频消融术治疗腰背部疼痛可提高疗效和PPDT、PPTO,减轻疼痛、应激反应,改善功能障碍,值得推广。Objective To explore the clinical effect of ultrasound-guided myofascial trigger point acupuncture combined with target radiofrequency ablation in the treatment of patients with low back pain.Methods A total of 80 patients with low back pain admitted from January 2021 to February 2022 were selected and randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with simple target radiofrequency ablation,and the observation group was treated with ultrasound-guided myofascial trigger point acupuncture combined with target radiofrequency ablation.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the pressure pain discomfort threshold(PPDT)and pressure pain tolerance threshold(PPTO)in the observation group were higher than those in the control group(P<0.05).After treatment,the scores of Pain Rating Index(PRI),Present Pain Intensity(PPI)and Oswestry Disability Index(ODI)in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of cortisol(Cor)and adrenocorticotrophic hormone(ACTH)in the observation group were lower than those in the control group(P<0.05).Conclusion Ultrasound-guided myofascial trigger point acupuncture combined with target radiofrequency ablation in the treatment of low back pain can improve the efficacy and PPDT,PPTO,reduce pain and stress response,and improve dysfunction,which is worthy of promotion.
