基于“一气周流”论治甲状腺结节

摘　　要：“一气周流”理论由清代医家黄元御提出,强调疾病发生的主要原因为脏腑气机不流通,其内涵可概括为“左路木火升发,右路金水敛降,中土之气斡旋”。本文基于此理论探讨甲状腺结节的辨治,认为中土虚衰,肺胃之气失于清降敛藏,肝脾之气失于温升健运,导致滞气、凝痰、瘀血壅遏颈前是病情发展的重要机制。治疗时应整体把握气机变化,以恢复中土斡旋为要,并结合四象的动态变化,运用健脾胜湿、清肺降胃、温肾和胃、疏肝清心等法行滞气、消凝痰、化瘀血,恢复气机周流不息,从而消除甲状腺结节及其不适,供临床用药参考。The theory of “circulating qi inside the body” was put forward by Huang Yuanyu,a physician in the Qing Dynasty.It emphasizes that the main reason for the occurrence of diseases is qi stagnation in the Zang-fu organs.Its connotation can be summarized as “liver qi and kidney qi ascend on the left movement path;lung qi and heart qi descend on the right movement path,and middle qi mediates”.Based on this theory,this paper discusses the differentiation and treatment of thyroid nodules.It is believed that the deficiency of middle qi,the loss of the lung qi and stomach qi due to the accumulation of Qing,and the loss of the liver qi and spleen qi due to the rise of temperature and healthy transport are the important mechanisms of the development of the disease,which lead to the qi stagnation,phlegm coagulation,and blood stasis of the neck.During the treatment,it is necessary to grasp the changes in the qi mechanism as a whole,take the restoration of middle qi mediation as the key point,and combine the dynamic changes of the four images,so as to apply the methods of invigorating spleen and eliminating dampness,clearing lung fire and lowering stomach fire,warming kidney and harmonizing stomach,and soothing liver and clearing heart to eliminate qi stagnation,phlegm coagulation,and blood stasis,restore the constant flow of qi mechanism,and relieve thyroid nodule and discomfort for clinical reference.
