摘  要:目的:探讨髌骨周围电灼部分去极化技术对膝关节单髁置换术后患者膝关节功能及髌骨功能的影响。方法:回顾性分析皖南医学院附属池州医院2020年3月至2022年10月膝关节内侧骨关节炎并进行单髁置换手术治疗的60例患者,将患者随机分为研究组及对照组,所有患者均采取单髁置换手术进行治疗,研究组在此基础上联合采取髌骨周围电灼部分去极化技术进行治疗。分别对两组术前及术后1月、术后3月、术后6月的疼痛情况、膝关节功能、髌骨倾斜角、髌骨评分(Feller评分)进行比较。结果:研究组术后1月(t=3.437,P=0.001)、术后3月(t=6.591,P<0.001)及术后6月(t=5.162,P<0.001)的视觉模拟评分(VAS)显著低于对照组;研究组术后1月(t=9.625,P<0.001)、术后3月(t=7.970,P<0.001)及术后6月(t=4.092,P<0.001)的美国特种外科医院膝关节评分(HSS)显著高于对照组;研究组术后1月(t=2.571,P=0.013)、术后3月(t=2.671,P=0.010)及术后6月(t=2.741,P=0.008)的髌骨倾斜角显著低于对照组;研究组术后1月(t=2.510,P=0.015)、术后3月(t=3.384,P=0.001)及术后6月(t=3.483,P=0.001)的Feller评分显著高于对照组。结论:髌骨周围电灼部分去极化技术可有效改善膝关节单髁置换术后患者膝关节功能及髌骨功能。Objective:To investigate effects of partial depolarization on peripatellar tissues underwent electrocautery on the post-operative knee joint and patellar functions of patients underwent unicompartmental knee arthroplasty(UKA)surgery.Methods:This retrospective study was conducted based on 60 knee osteoarthritis(OA)patients with UKA surgery in the Affiliated Chizhou Hospital of Wannan Medical College from March 2020 to October 2022.Patients were randomly assigned to the treatment group and control group.Patients in the control group received only UKA surgery,while patients in the treatment group received UKA surgery combined with partial depolarization of peripatellar tissue underwent electrocautery.Pain,knee joint function,patellar inclination angle and the Feller patellar score of the both groups were compared before and 1,3,6 months after surgery,respectively.Results:The Visual Analogue Scale scores of patients in the treatment group were significantly lower than the control group at 1 month(t=3.437,P=0.001),3 months(t=6.591,P<0.001)and 6 months(t=5.162,P<0.001)after surgery,respectively.The Hospital for Special Surgery Knee scores of patients in the treatment group were higher than the control group at 1 month(t=9.625,P<0.001),3 months(t=7.970,P<0.001)and 6 months(t=4.092,P<0.001)after surgery,respectively.The patellar inclination angles of patients in the study group were significantly smaller than the control group at 1 month(t=2.571,P=0.013),3 months(t=2.671,P=0.010)and 6 months(t=2.741,P=0.008)after surgery,respectively.Feller scores of patients in the study group were higher than the control group at 1 month(t=2.510,P=0.015),3 months(t=3.384,P=0.001)and 6 months(t=3.483,P=0.001)after surgery,respectively.Conclusion:Partial depolarization of peripatellar tissues by electrocautery can effectively improve the knee and patellar functions in OA patients after unicompartmental knee arthroplasty surgery.
