摘　　要：目的本研究旨在分析某三甲医院胃恶性肿瘤内科治疗DRG分组住院费用结构及影响患者住院费用因素,为提高医院运行效益、更好地控制住院费用提供参考。方法收集了某三甲医院2018年1月1日至2021年9月30日出院的胃恶性肿瘤内科治疗患者信息。应用描述性统计分析和多元逐步回归等统计学方法,对胃恶性肿瘤内科患者的住院费用结构及影响因素进行了分析,并提出了合理性建议。结果费用结构分析显示,药品费用占比最高(62.88%),而医疗费用占比最低(1.73%)。影响因素分析表明,性别、年龄、就诊次数、是否操作、治疗类型、住院天数、药品费用比例是影响住院费用的因素。结论医院可通过优化诊疗服务流程、推广日间化疗病房等方法合理降低平均住院日,加强治疗流程管理并进一步关注医务人员的技术劳动价值,同时还应加强科研力度,深入开展多学科综合治疗的效用评价研究,实现医疗成本的合理控制,进一步提高医院运营效率。Objective To analyzes the structure of DRG grouping hospitalization expenses and the factors affecting the hospitalization expenses of patients in the medical treatment of gastric malignancies in a third-class hospital,and puts forward reasonable suggestions to provide reference for improving the hospital operation efficiency and controlling the hospitalization expenses.Methods Collect the information of patients with gastric cancer treated in the department of internal medicine in a Class I1I hospital from January 1,2018 to September 30,2021,analyze the structure of hospitalization expenses and influencing factors of patients with gastric cancer treated in the department of internal medicine by using descriptive statistical analysis,multiple stepwise regression and other statistical methods,and put forward reasonable suggestions.Results According to the cost structure analysis,the proportion of drug expenses is the highest(62.88%),and the proportion of medical expenses is the lowest(1.73%);Analysis of influencing factors:gender,age,number of visits,whether to operate,treatment type,length of stay,proportion of drug expenses are the factors that affect hospitalization expenses.Conclusion The hospital can reasonably reduce the average hospital stay by optimizing the diagnosis and treatment service process,promoting the daytime chemotherapy ward and other methods,strengthen the treatment process management,and further pay attention to the technical labor value of medical staff.At the same time,it should also strengthen the scientific research efforts,carry out in-depth research on the effectiveness evaluation of multidisciplinary comprehensive treatment,and achieve reasonable control of medical costs,further improve operational efficiency.
