摘  要:目的研究无创呼吸机辅助通气对新生儿呼吸窘迫综合征(NRDS)患儿症状及血气指标的影响。方法选择2017年10月~2019年5月我院儿科收治的NRDS患儿86例,采用随机数余数分组法将其分为常规组与无创组各43例,常规组接受常规基础治疗+常规机械通气,无创组接受常规基础治疗+持续气道正压通气,观察比较两组患儿(1)治疗后治愈率、通气时间、住院时间及住院费用,(2)治疗前及治疗后1h、6h、24h血气指标[动脉血氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、氧合指数(OI)、吸入氧浓度(FiO_(2))],(3)并发症发生情况。结果无创组治愈率(93.02%)显著高于常规组(74.42%)(P<0.05);无创组通气时间、住院时间明显短于常规组,住院费用明显低于常规组(P<0.05);无创组治疗后各时间PaO_(2)值明显高于常规组,PaCO_(2)、FiO_(2)、OI值明显低于常规组(P<0.05);无创组总并发症发生率(6.98%)显著低于常规组(23.26%)(P<0.05)。结论无创呼吸机辅助通气治疗对NRDS患儿疗效确切,能有效改善患儿血气指标,促进通气功能,值得临床推广应用。Objective To investigate the effects of non-invasive ventilator-assisted ventilation on symptoms and blood gas parameters in children with neonatal respiratory distress syndrome(NRDS).Methods 86 NRDS children admitted to our hospital from October 2017 to May 2019 were enrolled,and divided into two groups by random number table methods,each with 43 cases.The routine group received routine basic treatment and routine mechanical ventilation,while the non-invasive group received routine basic treatment and continuous positive airway pressure ventilation.The cure rate,ventilation time,hospitalization time,hospitalization expenses,blood gas indexes[partial arterial oxygen pressure(PaO_(2)),partial carbon dioxide pressure(PaCO_(2)),oxygenation index(OI),inhalation oxygen concentration(FiO_(2))],and complications were observed and compared between the two groups.Results The cure rate of the non-invasive group was significantly higher than that of the routine group(93.02%vs 74.42%,P<0.05).The ventilation time and hospital stay of the non-invasive group were significantly shorter than those of the routine group,and the hospitalization cost was significantly lower than that of the routine group(P<0.05);The values of PaO_(2) at 24h after treatment in the two groups were significantly higher than those at 1h after treatment,and the values of PaCO_(2),FiO_(2),and OI at 24h after treatment were significantly lower than at 1h after treatment(P<0.05);The incidence of total complications was significantly lower in the non-invasive group than in the routine group(6.98%vs 23.26%,P<0.05).Conclusion Application of non-invasive ventilator-assisted ventilation can effectively improve the blood gas index,and promote ventilation for NRDS children,which is worthy of promotion.
