气压治疗仪联合穴位艾灸在脑梗死偏瘫病人LEDVT预防中的护理效果探讨

摘　　要：目的分析气压治疗仪联合穴位艾灸在脑梗死偏瘫病人急性下肢深静脉血栓形成(LEDVT)预防中的护理效果。方法选择医院2019年6月至2021年6月诊治的脑梗死偏瘫患者86例。使用计算机随机数分组法将所有患者分成对照组和干预组,例数均为43例。对照组患者接受气压治疗仪干预,干预组患者接受气压治疗仪联合穴位艾灸干预。比较2组患者运动能力(FMA)及平衡能力(Berg)评分、LEDVT及护理不良事件发生情况、下肢股静脉血流峰速。结果干预前1d,2组患者FMA及Berg评分差异比较无统计学意义(P>0.05)。干预结束1d,干预组患者FMA及Berg评分高于对照组(P<0.05)。干预组患者LEDVT发生率低于对照组(P<0.05);护理不良事件发生率与对照组比较差异无统计学意义(P>0.05)。干预前1d,2组患者下肢股静脉血流峰速差异比较无统计学意义(P>0.05)。干预结束1d,干预组患者下肢股静脉血流峰速大于对照组(P<0.05)。结论气压治疗仪联合穴位艾灸用于脑梗死偏瘫患者护理中能减少LEDVT发生率,改善运动和平衡能力,促使下肢股静脉血流加速,建议使用。Objective To analyze the nursing effect of air pressure treatment apparatus combined with acupoint moxibustion in the prevention of lower extremity deep venous thrombosis(LEDVT)in hemiplegic patients with cerebral infarction.Methods Eighty-six patients with cerebral infarction and hemiplegia in our hospital from June 2019 to June 2021 were enrolled,and randomly divided into two groups by computer,each with 43 cases.Control group received interventions via air pressure treatment apparatus,while intervention group received air pressure treatment apparatus combined with acupoint moxibustion.Then comparison was conducted on the Fugl-Meyer Assessment(FMA),the Berg Balance Scale(BBS)score,LEDVT rate,nursing related adverse events and blood flow velocity of lower extremity femoral vein.Results Scores of FMA and BBS at 1d before intervention yielded no statistical difference between two groups(P>0.05),while intervention group scored remarkably higher on FMA and BBS than control group at 1d after intervention(P<0.05).The incidence rate of LEDVT was lower in intervention group than in control group(P<0.05),and no statistical difference was found in the rate of nursing related adverse events(P>0.05).The blood flow velocity of lower extremity femoral vein demonstrated no statistical difference between two groups at 1d before intervention(P>0.05),while the velocity was significantly increased in intervention group compared with control group at 1d after intervention(P<0.05).Conclusion Application of air pressure treatment apparatus combined with acupoint moxibustion can effectively reduce the LEDVT rate,improve motor and balance abilities,and increase the blood flow velocity of lower extremity femoral vein in hemiplegic patients with cerebral infarction,which is worthy of recommendation.
