摘　　要：目的探讨纤维蛋白原与白蛋白比值(FAR)、肾小球滤过率(eGFR)、糖化血红蛋白(HbA1c)、胱抑素-C(Cys-C)对经皮冠脉介入术(PCI)后造影剂肾病(CAN)的预测价值。方法选取2020年2月至2022年10月于医院接受PCI治疗的86例急性心肌梗死患者作为研究对象,根据术后是否发生CAN分为CAN组(25例)和非CAN组(61例),对比FAR、eGFR、HbA1c、Cys-C;采用logistic回归模型建立FAR、eGFR、HbA1c、Cys-C联合预测模型;采用受试者工作特征(ROC)曲线分析FAR、eGFR、HbA1c、Cys-C及四项联合预测并发CAN的曲线下面积(AUC)、灵敏度及特异度。结果CAN组FAR、HbA1c、Cys-C高于非CAN组,eGFR低于非CAN组(P<0.05)。ROC曲线分析显示,FAR、eGFR、HbA1c、Cys-C及四项联合预测行PCI患者术后并发CAN的AUC分别为0.617、0.766、0.744、0.669、0.929(P<0.05)。结论FAR、eGFR、HbA1c、Cys-C在PCI术后并发CAN患者中呈高表达,检测其变化对预测术后CAN的发生具有重要意义。Objective To explore the predictive value of fibrinogen to albumin ratio(FAR),glomerular filtration rate(eGFR),glycosylated hemoglobin(HbA1c),cystatin C(Cys-C)on contrast associated nephropathy(CAN)after percutaneous coronary intervention(PCI).Methods A total of 86 patients with acute myocardial infarction who underwent PCI in the hospital from February 2020 to October 2022 were selected as the study subjects.They were divided into CAN group(25 cases)and non-CAN group(61 cases)based on whether postoperative CAN occurred.FAR,eGFR,HbA1c,Cys-C were compared.Using logistic regression models to establish a joint prediction model for FAR,eGFR,HbA1c,Cys-C.The receiver operating characteristic(ROC)model was used to analyze the area under the curve(AUC)value,sensitivity and specificity of FAR,eGFR,HbA1c,Cys-C and the combination of four factors in predicting contrast nephropathy after PCI.Results The FAR,HbA1c,Cys-C of the CAN group were higher than those of the non-CAN group,while eGFR was lower than that of the non-CAN group(P<0.05).The ROC analysis showed that the AUC values of FAR,eGFR,HbA1c,Cys-C and the four combined predictors of contrast agent nephropathy after PCI were 0.617,0.766,0.744,0.669,0.929,respectively(P<0.05).Conclusion FAR,eGFR,HbA1c and Cys-C are highly expressed in patients with CAN after PCI,and detecting their changes is of great significance in predicting the occurrence of postoperative contrast-enhanced nephropathy.
