摘  要:目的探讨枸橼酸咖啡因联合加温湿化高流量鼻导管通气(HHHFNC)治疗早产儿呼吸窘迫综合征(RDS)的效果,及其对脑血流、血氧的影响。方法本研究为前瞻性研究。选取2019年5月至2022年1月驻马店市中心医院诊治的82例RDS早产儿作为研究对象,按照随机数字表法分为对照组(HHHFNC治疗)与观察组(枸橼酸咖啡因联合HHHFNC治疗),各41例,对比双侧大脑前、中、后动脉及基底动脉收缩期峰值流速(Vs)、舒张末期流速(Vd)、血流阻力指数(RI)及血氧分压(PaO_(2))、pH、二氧化碳分压(PaCO_(2))、撤机成功率、无创辅助通气时间。结果观察组治疗前后大脑前动脉、大脑中动脉、大脑后动脉、基底动脉的Vs、Vd、RI差值均高于对照组(P<0.05)。观察组治疗前后PaO_(2)、pH、PaCO_(2)差值均高于对照组(P<0.05)。观察组撤机成功率高于对照组,而无创辅助通气时间短于对照组(P<0.05)。结论枸橼酸咖啡因治疗RDS早产儿效果较好,有利于改善脑血流参数和血氧指标。Objective To investigate the effect of caffeine citrate combined with warm humidification and high flow nasal catheter ventilation(HHHFNC)on premature infants with respiratory distress syndrome(RDS)and its effects on cerebral blood flow and blood oxygen.Methods This was a prospective study.A total of 82 premature infants with respiratory distress syndrome diagnosed and treated in Zhumadian Central Hospital from May 2019 to January 2022 were selected as research objects.They were divided into control group(HHHFNC treatment)and observation group(caffeine citrate combined with HHHFNC treatment)according to random number table method,with 41 cases in each group.The peak systolic flow rate(Vs),end-diastolic flow rate(Vd),blood flow resistance index(RI),partial oxygen pressure(PaO_(2)),pH,partial carbon dioxide pressure(PaCO_(2)),success rate of withdrawal,and time of non-invasive auxiliary ventilation were compared in bilateral cerebral arteries,middle and posterior arteries and basilar arteries.Results The difference values of Vs,Vd and RI of anterior cerebral artery,middle cerebral artery,posterior cerebral artery and basilar artery in observation group were higher than those in control group before and after treatment(P<0.05).The difference values of PaO_(2),pH and PaCO_(2) in observation group were higher than those in control group before and after treatment(P<0.05).The success rate of withdrawal in observation group was higher than that in control group,and the duration of non-invasive assisted ventilation was shorter than that in control group(P<0.05).Conclusion Caffeine citrate has a good effect in the treatment of premature infants with respiratory distress syndrome,which is beneficial to improve cerebral blood flow parameters and blood oxygen indexes.
