中极、三阴交烧山火针刺法联合低频脉冲电刺激治疗产后尿潴留的效果  

Effect of Zhongji and Sanyinjiao Shaoshan Fire Acupuncture Combined with Low Frequency Pulse Electrical Stimulation in Postpart Umurinary Retention

摘　　要：目的探讨中极、三阴交烧山火针刺法联合低频脉冲电刺激治疗产后尿潴留(PUR)的效果。方法前瞻性研究,纳入2021年2月至2022年6月医院收治的120例PUR患者,以随机数字表法分为针刺组(40例,中极、三阴交烧山火针刺法治疗)、低频脉冲电刺激组(40例,低频脉冲电刺激治疗)和针刺联合低频脉冲电刺激组(40例,中极、三阴交烧山火针刺法+低频脉冲电刺激治疗),共治疗3 d。比较3组临床疗效、排尿情况、盆底肌力(盆底Ⅰ类及Ⅱ类肌纤维收缩时间)。结果针刺组、低频脉冲电刺激组总有效率低于针刺联合低频脉冲电刺激组(P<0.05);与针刺组、低频脉冲电刺激组比较,针刺联合低频脉冲电刺激组排尿后膀胱残余尿量少,首次排尿时间短(P<0.05);治疗后,针刺组、低频脉冲电刺激组盆底Ⅰ类及Ⅱ类肌纤维收缩时间短于针刺联合低频脉冲电刺激组(P<0.05)。结论PUR患者采用中极、三阴交烧山火针刺法联合低频脉冲电刺激治疗的效果显著,可有效改善盆底肌力,降低膀胱残余尿量,缩短首次排尿时间。Objective To explore the efficacy of Zhongji and Sanyinjiao Shaoshan fire acupuncture combined with low frequency pulse electrical stimulation in the treatment of postpart umurinary retention(PUR).Methods This study was a prospective study that included 120 PUR patients admitted to the hospital from February 2021 to June 2022.They were divided into acupuncture group(40 cases,treated with Zhongji and Sanyinjiao Shaoshan fire acupuncture),low-frequency pulse electrical stimulation group(40 cases,treated with low frequency pulse electrical stimulation),and acupuncture combined with low-frequency pulse electrical stimulation group(40 cases,treated with Zhongji and Sanyinjiao Shaoshan fire acupuncture+low-frequency pulse electrical stimulation)using a random number table method,for a total of 3 days of treatment.The clinical efficacy,urination status and pelvic floor muscle strength(pelvic floor typeⅠandⅡmuscle fiber contraction time)among three groups were compared.Results The total effective rate of the acupuncture group and the low-frequency pulse electrical stimulation group was lower than that of the acupuncture combined low-frequency pulse electrical stimulation group(P<0.05).Compared with the acupuncture group and the low-frequency pulse electrical stimulation group,the acupuncture combined with low-frequency pulse electrical stimulation group had less residual urine in the bladder after urination and shorter first urination time(P<0.05).After treatment,the contraction time of Class I and II pelvic floor muscle fibers in the acupuncture group and low-frequency pulse electrical stimulation group was shorter than that in the acupuncture combined with low-frequency pulse electrical stimulation group(P<0.05).Conclusion The combination of Zhongji and Sanyinjiao Shaoshan fire acupuncture and low-frequency pulse electrical stimulation has a significant effect on PUR patients,which can effectively improve pelvic floor muscle strength,reduce residual urine volume in the bladder,and shorten the first urination time.
