基于中医传承辅助平台的迟莉丽教授治疗溃疡性结肠炎用药规律研究

摘　　要：目的:研究探讨迟莉丽教授治疗溃疡性结肠炎的学术思想与用药规律,以期为相关临床研究提供参考。方法:收集迟莉丽教授诊治溃疡性结肠炎的医案,录入中医传承辅助平台软件,利用频数统计、关联规则算法、复杂系统熵聚类等数据挖掘技术,对迟莉丽教授治疗溃疡性结肠炎的用药规律进行深入挖掘。结果:本研究共纳入患者159例,共566诊次,核心药物为薏苡仁、茯苓、白芍、木香、甘草、黄连等,多归脾、胃、肝经,寒温药比例相当,药物关联分析得出关联系数>0.06的药对17对;聚类分析得出核心药物组合20组、新方9首。结论:溃疡性结肠炎病机复杂,“虚”“毒”“瘀”“郁”并存,迟莉丽教授治疗溃疡性结肠炎重在健脾益气以固本扶正,清热解毒、活血化瘀以祛邪缓急,疏肝解郁与宁心安神并举以调畅情志,寒温并用,攻补兼施,用药平和,配伍精当。本研究基于中医传承辅助平台总结迟莉丽教授临证运用中药治疗溃疡性结肠炎的用药规律,为临床诊治提供直观、客观及可视化的依据,有利于传承其宝贵的临床经验和学术精华。未来将继续扩大验案数量,并从药-症、药-证关联等多维度、多方面进行深入挖掘分析。Objective:To summarize the academic thought and medication rule of Professor CHI Lili in the treatment of ulcerative colitis,in order to provide reference for related clinical research.Methods:Professor CHI Lili’s medical records on the diagnosis and treatment of ulcerative colitis were collected and input into the software of Traditional Chinese Medicine Inheritance Support System Platform V2.5.Professor CHI Lili’s medication rules in the treatment of ulcerative colitis was analyzed based on data mining techniques such as frequency statistics,association rule algorithm,complex system entropy clustering,etc.Results:A total of 159 patients were treated with 566 visits.The core drugs on ulcerative colitis were Yiyiren(Semen Coicis),Fuling(Poria),Baishao(Radix Paeoniae Alba),Muxiang(Radix Aucklandiae),Gancao(Radix Glycyrrhizae),Huanglian(Rhizoma Coptidis).Most of them returned to the spleen,stomach and liver meridians,and the proportion of cold and warm drugs was the same.Drug correlation analysis showed that there were 17 pairs of drugs with correlation coefficient greater than 0.06,cluster analysis showed that there were 20 core drug combinations and 9 new prescriptions.Conclusion:The pathogenesis of the ulcerative colitis is complex,with the coexistence of deficiency,poison,stasis and depression.In the treatment of ulcerative colitis,Professor CHI Lili focuses on the methods of invigorating the spleen and replenishing Qi(气),clearing heat and detoxification,promoting blood circulation and removing blood stasis.Dispersing stagnated liver Qi and soothing the heart and tranquilizing the mind are paid attention to at the same time.The cold medicine and warm medicine were used jointly;the attack method and tonifying method were combined to use,the drugs are mild in medicinal property;and the compatibility of medicines is appropriate.Based on Traditional Chinese Medicine Inheritance Support System Platform,this study summarizes Professor CHI Lili’s rule of clinical use of TCM medicines in the treatment of ulcera
