摘　　要：目的 探讨结核感染T细胞检测(T-SPOT.TB)在菌阴性肺结核诊断中的应用价值。方法 选择2019年1月—2021年1月韶关市粤北第二人民医院诊断为菌阴性肺结核的100例患者以及同时间段内在本院就诊的100例非结核肺部疾病患者的数据资料进行回顾性分析。描述菌阴性肺结核组患者的初次治疗分布情况以及非结核肺部疾病组患者的疾病分布情况。分析T-SPOT.TB对菌阴性肺结核患者的诊断价值。结果 TSPOT.TB对菌阴性肺结核患者的诊断灵敏度为86.00%(86/100),诊断特异度为95.00%(95/100),诊断准确度为90.50%(181/200),诊断的阳性预测值为94.51%(86/91),诊断的阴性预测值为87.16%(95/109)。结论 TSPOT.TB的检测方法对于菌阴性肺结核患者具有较为良好的诊断效能。Objective To explore the application value of TB infection T cell detection(T-SPOT.TB)in the diagnosis of bacteria negative pulmonary tuberculosis.Methods The data of 100 patients diagnosed with bacterial negative pul-monary tuberculosis and 100 patients with non-tuberculosis pulmonary disease in North Guangdong Second People's Hospital from January 2019 to January 2021 were selected for retrospectively analyzed.The distribution of initial treat-ment in patients with bacterial negative tuberculosis and the distribution of disease in patients with non-tuberculous lung disease were described.The diagnostic value of T-SPOT.TB in patients with bacteria-negative pulmonary tuber-culosis was analyzed.Results The diagnostic sensitivity of T-SPOT.TB for bacterial negative pulmonary tuberculosis patients was 86.00%(86/100),the diagnostic specificity was 95.00%(95/100),the diagnostic accuracy was 90.50%(181/200),the positive predictive value of diagnosis was 94.51%(86/91),and the negative predictive value of diagno-sis was 87.16%(95/109).Conclusion The detection method of T-SPOT.TB has better diagnostic efficacy in patients with bacteria negative tuberculosis.
