尿β2-微球蛋白、尿微量白蛋白/肌酐结合血清胱抑素C检测在早期糖尿病肾病中的应用

摘　　要：目的 探究对早期糖尿病肾病患者采用尿β2-微球蛋白(β2-microglobulin, β2-MG)、尿微量白蛋白(microalbumin, mALB)/肌酐(creatinine, CREA)结合血清胱抑素C(cystatin-C, Cys-C)检测的临床应用价值。方法 选取2022年10月—2023年8月宜兴市中医医院收治的47例早期糖尿病肾病患者为研究组,同期选择糖尿病患者46例为对照1组,再选择同时间段健康体检患者46例为对照2组。比较3组患者β2-MG、mALB/CREA比值、Cys-C水平;比较联合以及单独检测的诊断结果以及诊断效能,包括诊断准确度、敏感度、特异度。结果 研究组β2-MG、mALB/CREA、Cys-C均高于对照1组和对照2组,差异有统计学意义(P<0.05)。联合检测真阳性46例,真阴性86例;单独β2-MG检测真阳性33例,真阴性74例;单独mALB/CREA比值检测真阳性37例,真阴性76例;单独Cys-C检测真阳性32例,真阴性72例。联合检测β2-MG、mALB/CREA比值、Cys-C的诊断准确度(94.96%)、敏感度(97.87%)、特异度(93.48%)高于各指标单独检测,差异有统计学意义(χ^(2)=18.641、13.406、6.900、12.413、8.340、5.163、21.989、14.763、8.779,P<0.05)。结论 在早期糖尿病肾病患者中,其β2-MG、mALB/CREA比值、Cys-C水平均较高,联合检测后诊断准确度、敏感度、特异度均较高。Objective Explore the use of urine in patients with early diabetes nephropathyβ2-Microglobulin(β2-mi-croglobulin,βThe clinical application value of 2-MG,microalbumin(mALB)/creatinine(CREA)combined with serum cystatin-C(Cys-C)detection.Methods A total of 47 patients with early diabetic nephropathy admitted to Yixing Hos-pital of Traditional Chinese Medicine from October 2022 to August 2023 were selected as the study group.At the same time,46 patients with diabetes were selected as the control group 1,and 46 patients with physical examination at the same time were selected as the control group 2.The levels ofβ2-MG,mALB/CREA ratio and Cys-C were com-pared among the three groups.The diagnostic results and diagnostic efficacy of combined and single detection were compared,including diagnostic accuracy,sensitivity and specificity.Results The levels ofβ2-MG,mALB/CREA and Cys-C in the study group were higher than those in the control group 1 and the control group 2,and the differences were statistically significant(P<0.05).Combined detection was true positive in 46 cases and true negative in 86 cases.There were 33 true positive cases and 74 true negative cases detected byβ2-MG alone.The single mALB/CREA ratio was true positive in 37 cases and true negative in 76 cases.Cys-C alone was true positive in 32 cases and true nega-tive in 72 cases.The diagnostic accuracy(94.96%),sensitivity(97.87%)and specificity(93.48%)of combined detec-tion ofβ2-MG,mALB/CREA ratio and Cys-C were higher than those of each index alone,and the difference was sta-tistically significant(χ^(2)=18.641),13.406,6.900,12.413,8.340,5.163,21.989,14.763,8.779,P<0.05).Conclusion In patients with early diabetes nephropathyβThe 2-MG,mALB/CREA ratio,and Cys-C levels are all high,and the combined detection has a high diagnostic accuracy,sensitivity,and specificity.
