研究血清PA、CRP、PCT水平联合检测在老年细菌性肺炎患者的诊断价值

摘　　要：目的 评估老年细菌性肺炎患者应用血清前白蛋白(prealbumin, PA)、C反应蛋白(C-reactive protein,CRP)、血小板(platelet, PCT)水平联合检测的有效性。方法 选择2022年8月—2023年9月无锡市康复医院接诊的50例老年肺炎患者为研究对象,按照是否感染细菌分为细菌感染组和非细菌感染组,每组25例。选择同时期健康体检者50例作为对照组,均提供血清PA、CRP、PCT水平联合检测,分析应用价值。结果 临床确诊的阳性检出率为25.00%,PA检测的阳性率为20.00%,PCT检测的阳性率为20.00%,CRP检测的阳性率为11.00%,联合检测的阳性检出率为24.00%。联合检测的敏感度、特异度、准确性高于PA检测、PCT检测,联合检测的敏感度96.00%、准确性99.00%高于CRP检测,差异有统计学意义(χ2=20.053、18.000,P<0.05)。联合检测的特异度与CRP检测比较,差异无统计学意义(P>0.05)。结论 老年细菌性肺炎患者应用血清PA、CRP、PCT水平联合检测具有较高的应用效能。Objective To evaluate the effectiveness of combined detection of serum prealbumin(PA),C-reactive pro-tein(CRP)and platelet(PCT)levels in elderly patients with bacterial pneumonia.Methods 50 elderly patients with pneumonia treated in Wuxi Rehabilitation Hospital from August 2022 to September 2023 were selected as the study objects.According to whether they were infected with bacteria,they were divided into bacterial infection group and non-bacterial infection group,with 25 cases in each group.50 healthy subjects were selected as the control group,and the serum levels of PA,CRP and PCT were detected jointly to analyze the application value.Results The positive de-tection rate of clinical diagnosis was 25.00%,that of PA detection was 20.00%,that of PCT detection was 20.00%,that of CRP detection was 11.00%,and that of combined detection was 24.00%.The sensitivity,specificity and accu-racy of combined detection were higher than those of PA and PCT,and the sensitivity and accuracy of combined detec-tion were 96.00%and 99.00%,respectively,than those of CRP detection,the difference was statistically significant(χ^(2)=20.053,18.000,P<0.05).There was no significant difference between the specificity of combined detection and CRP detection(P>0.05).Conclusion The combined detection of serum PA,CRP and PCT levels in elderly patients with bacterial pneumonia has high application efficiency.
