慢性鼻窦炎-鼻息肉术后复发的影响因素分析

摘　　要：目的 阐明慢性鼻窦炎-鼻息肉(chronic rhinosinusitis with nasal polyp, CRSwNP)术后复发的影响因素。方法 回顾性分析2021年1月—2022年6月在广东省韶关市粤北人民医院耳鼻喉科行慢性鼻窦炎-鼻息肉术治疗的333例慢性鼻窦炎伴鼻息肉患者的临床资料,根据术后复发情况分为复发组(n=138)和未复发组(n=195),采用多因素Logistic回归分析影响CRSw NP患者术后1年复发的危险因素。结果 两组患者性别、年龄、病程、文化程度比较,差异无统计学意义(P>0.05);两组患者饮酒、吸烟、切除情况、手术时间比较,差异有统计学意义(P<0.05)。Logistic回归分析显示,饮酒、吸烟、切除情况、手术时间均为CRSwNP患者术后1年内复发的危险因素(OR=2.661、1.521、1.483、1.365,P<0.05)。结论 饮酒、吸烟、切除情况、手术时间是慢性鼻窦炎-鼻息肉术后复发的影响因素,对此需针对此类复发因素采取对应处理措施,减少复发。Objective To elucidate the influencing factors of postoperative recurrence of chronic sinusitis with nasal polyp.Methods Clinical data of 333 patients with chronic sinusitis with nasal polyps treated by chronic sinusitis and nasal polyps in the Otolaryngology Department of North Guangdong People's Hospital,Shaoguan City,Guangdong Province from January 2021 to June 2022 were analyzed retrospectively.According to postoperative recurrence,they were divided into recurrence group(n=138)and non-recurrence group(n=195).Multivariate Logistic regression was used to analyze the risk factors for recurrence in patients with CRSwNP 1 year after surgery.Results There were no statistically significant differences in gender,age,course of disease and educational level between the two groups(P>0.05).The two groups were compared in terms of alcohol consumption,smoking,resection,and operation time,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that drinking,smoking,resec-tion situation and operation time were all risk factors for recurrence in CRSwNP patients within 1 year after surgery(OR=2.661,1.521,1.483,1.365,P<0.05).Conclusion Drinking,smoking,excision and operation time are the factors affecting the recurrence of chronic sinusitis and nasal polyps after operation,and corresponding measures should be taken to reduce the recurrence of these factors.
