摘　　要：目的探究多囊卵巢综合征(PCOS)患者血清内毒素、Toll样受体4(TLR4)的表达水平和临床意义。方法选取2021年3月至12月唐山市妇幼保健院收治的50例PCOS伴有月经紊乱或既往检测排卵提示稀发排卵甚至排卵障碍的患者作为研究组,选取同期同一医院体检的50例非PCOS女性作为对照组。采用酶联免疫吸附试验(ELISA)法检测两组血清内毒素、TLR4水平;绘制受试者工作特征(ROC)曲线分析血清内毒素、TLR4水平对PCOS的诊断价值。结果与对照组相比,研究组PCOS患者体重指数(BMI)、血清内毒素、TLR4、睾酮(T)、黄体生成素(LH)、胰岛素抵抗指数(HOMA-IR)水平显著升高(P<0.05)。血清内毒素、TLR4诊断PCOS的曲线下面积(AUC)优于二者单独诊断(Z_(二者联合-内毒素)=1.992、Z_(二者联合-TLR4)=2.814,P<0.05)。结论PCOS患者血清中内毒素、TLR4水平显著升高,二者联合检测对PCOS有较高的诊断价值。Objective To investigate the expression levels and clinical significance of serum endotoxin and Toll like receptor 4(TLR4)in patients with polycystic ovary syndrome(PCOS).Methods A total of 50 cases PCOS patients with menstrual disorder or previous ovulation detection indicating rare ovulation or even ovulation disorder admitted to Tangshan Maternal and Child Health Hospital from March to December 2021 were selected as the study group,and 50 non-PCOS patients who underwent physical examination in the same hospital during the same period were selected as the control group.The levels of serum endotoxin and TLR4 were detected by enzyme linked immunosorbent assay(ELISA).Receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of serum endotoxin and TLR4 levels in PCOS.Results Compared with the control group,the levels of body mass index(BMI),serum endotoxin,TLR4,testosterone(T),luteinizing hormone(LH)and homeostatic model assessment of insulin resistance(HOMA-IR)in PCOS patients in the study group were significantly increased(P<0.05).The area under the curve(AUC)of serum endotoxin and TLR4 in the diagnosis of PCOS was better than that diagnosed by them alone(Z_(combination-endotoxin=1.992),Z_(combination-TLR4)=2.814,P<0.05).Conclusions The serum levels of endotoxin and TLR4 increase significantly in PCOS patients.The combined detection of endotoxin and TLR4 has a high diagnostic value for PCOS.
