摘 要:目的探讨非药物干预对减轻新生儿桡动脉采血所致疼痛的效果。方法采取时序分组法,选取114例患儿分为3组:口服葡萄糖液组(37例)、口服葡萄糖液联合安抚奶嘴组(39例)和口服葡萄糖液联合音乐组(38例)。患儿进行桡动脉采血时,采用疼痛评估工具对疼痛程度进行评估,并观察穿刺前、穿刺中、穿刺后的心率、血氧饱和度、疼痛评分和啼哭时间。结果联合音乐组和联合安抚奶嘴组在疼痛评分和啼哭时间上均低于口服葡萄糖液组(P<0.05);联合音乐组的疼痛评分和啼哭时间低于联合安抚奶嘴组(P<0.05);3组在穿刺中和穿刺后的心率与血氧饱和度变化差异具有统计学意义(P<0.05)。结论新生儿实施桡动脉采血过程中进行非药物联合干预,有助于降低患儿对疼痛刺激的反应,减轻疼痛,缩短啼哭时间,且联合音乐组的效果最为显著,值得在临床上推广应用。Objective To investigate the effect of non-pharmacological interventions on reducing pain caused by radial arterial blood sampling in newborns.Methods A total of 114 newborns were divided into three groups with routine nested nursing care by the time series classification,including oral glucose group(n=37),oral glucose with pacifier group(n=39),and oral glucose with white noise group(n=38),and received oral glucose solution,oral glucose solution combined pacifier soothing,oral glucose solution combined with listening to the white noise,respectively,during radial arterial blood sampling.The pain level was assessed using the pain assessment tool.Pre-,intra-and post-puncture heart rate,oxygen saturation,pain score,and crying time were measured.Results The oral glucose group had statistically higher average pain score and longer crying time than the other two groups(P<0.05).The oral glucose with white noise group had statistically lower average pain score and shorter crying time than oral glucose with pacifier group(P<0.05).Three groups showed statistically significant differences in intra-and post-puncture heart rate and blood oxygen saturation changes(P<0.05).Conclusion The implementation of combined non-pharmacological interventions during radial arterial blood sampling for newborns is helpful to reduce the puncture-related pain by reducing their pain response to the puncture,and shorten the crying time.And the interventional effect of oral glucose solution combined with listening to the white noise is the most significant,which is worth promoting in clinical practice.
