摘 要:目的:探讨超声心动图结合血清高迁移率族蛋白B1、脑钠尿肽在新生儿持续性肺动脉高压诊断中的应用价值。方法:选取2020年1月—2022年1月普洱市人民医院收治的持续性肺动脉高压新生儿60例作为对照组,同期选取健康新生儿60例作为研究组。两组均进行超声心动图及高迁移率族蛋白B1、脑钠尿肽检测,比较两组上述指标检测结果及阳性率。结果:研究组左房内径、右房内径、左室内径、右室内径、肺动脉内径均大于对照组,差异有统计学意义(P<0.001)。研究组高迁移率族蛋白B1和脑钠尿肽水平均高于对照组,差异有统计学意义(P<0.001)。研究组左房内径、右房内径、左室内径、右室内径、肺动脉内径阳性率均高于对照组,差异有统计学意义(P<0.001)。研究组高迁移率族蛋白B1和脑钠尿肽阳性率均高于对照组,差异有统计学意义(P<0.001)。结论:新生儿持续性肺动脉高压患儿超声心动图指标、高迁移率族蛋白B1水平、脑钠尿肽水平均会出现异常,临床可以上述指标作为新生儿持续性肺动脉高压诊断、治疗效果评价、预后评估的重要参考依据。Objective:To investigate the application value of echocardiography combined with serum high mobility group protein B1 and brain natriuretic peptide in the diagnosis of neonatal persistent pulmonary hypertension.Methods:Sixty neonates with per⁃sistent pulmonary hypertension treated in Pu'er People's Hospital from January 2020 to January 2022 were selected as the control group,and sixty healthy neonates were selected as the study group during the same period.Echocardiography and detection of high mobility group protein B1 and brain natriuretic peptide were performed in both groups.The detection results and positive rates of the above indexes were compared between the two groups.Results:The left atrial diameter,right atrial diameter,left ven⁃tricular diameter,right ventricular diameter and pulmonary artery diameter in the study group were greater than those in the control group,and the difference was statistically significant(P<0.001);The levels of high mobility group protein B1 and brain natriuretic peptide in the study group were higher than those in the control group,with statistical significance(P<0.001);The positive rates of left atrial diameter,right atrial diameter,left ventricular diameter,right ventricular diameter and pulmonary artery diameter in the study group were higher than those in the control group,and the difference was statistically significant(P<0.001);The positive rates of high mobility group protein B1 and brain natriuretic peptide in the study group were higher than those in the control group,the difference was statistically significant(P<0.001).Conclusion:Abnormal echocardiographic indexes,high mobility group protein B1 level,and brain natriuretic peptide level in neonates with persistent pulmonary hypertension can be used as an important reference for diagnosis,treatment effect evaluation,and prognosis evaluation of neonates with persistent pulmonary hypertension.
