摘　　要：目的:探讨导管专项护理管理在老年患者非计划性拔管中的临床应用价值。方法:选取2021年1月—2023年3月收治的老年住院且留置相关导管者400例为观察对象,按照随机数字表法分为对照组与观察组,各200例。对照组实施常规护理干预,观察组实施导管专项管理。比较两组患者置管例数、非计划性拔管例数,分析两组护理管理指标落实情况,统计两组非计划性拔管来源。结果:观察组非计划性拔管例数低于对照组,差异有统计学意义(P=0.043);观察组风险评估、标识清晰和固定牢固的落实例数高于对照组,差异有统计学意义(P<0.001);观察组非计划性拔管仅1例,为非计划拔除胃管;对照组非计划性拔管来源分别为胃管6例,深静脉导管和尿管各1例。结论:针对老年住院留置导管者实施导管专项护理管理,可降低非计划性拔管发生率,更好地落实护理导管管理指标。Objective:To explore the clinical application value of catheter specialized nursing management in unplanned extubation of elderly patients.Methods:Four hundred cases of elderly hospitalized with indwelling relevant catheters admitted from January 2021 to March 2023 were selected as observation subjects,and were divided into the control group and the observation group according to the method of randomized numerical table,each of which consisted of two hundred cases.The control group implemented routine nursing intervention,and the observation group implemented catheter specialized management.Compare the number of cases of catheterization and unplanned extubation in the two groups,analyze the implementation of nursing management indexes in the two groups,and count the sources of unplanned extubation in the two groups.Results:The number of unplanned extubation cases in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.043);The number of cases with risk assessment,clear identification,and firm fixation in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.001);There was only one case of unplanned removal of gastric tubes in the observation group;The control group had 6 cases of unplanned extubation from gastric catheters,deep venous catheter and urinary catheter were found in 1 case.Conclusion:The implementation of catheter specialized nursing management for elderly hospitalized indwelling catheters can reduce the incidence of unplanned extubation and better implement nursing catheter management indicators.
