股骨近端防旋髓内钉与Gamma钉内固定治疗股骨粗隆间骨折的疗效比较

摘　　要：目的:探究股骨近端防旋髓内钉(PFNA)与Gamma钉内固定治疗股骨粗隆间骨折的疗效比较。方法:选取2021年5月—2023年5月于广州市南沙区第二人民医院收治的股骨粗隆间骨折患者60例为研究对象,随机分为PFNA组与Gamma组,各30例。Gamma组患者应用Gamma钉内固定,PFNA组患者应用PFNA内固定。比较两组围术期指标(手术时间、术中出血量、下地负重时间、骨折愈合时间)、疗效优良率、髋关节功能Harris评分及术后并发症情况。结果:PFNA组手术时间、骨折愈合时间均短于Gamma组,术中出血量少于Gamma组,下地负重时间长于Gamma组,差异有统计学意义(P<0.001);PFNA组疗效优良率高于Gamma组,差异有统计学意义(P=0.044);治疗后,两组Harris评分均升高,PFNA组高于Gamma组,差异有统计学意义(P<0.001);PFNA组术后并发症发生率低于Gamma组,差异有统计学意义(P=0.023)。结论:股骨粗隆间骨折通过PFNA内固定治疗的疗效优于Gamma钉内固定,且术后并发症轻微,髋关节功能恢复佳,利于缩短骨折愈合时间,值得推广及深入探究。Objective:To investigate the comparative efficacy of proximal femoral anti-rotation intramedullary nail(PFNA)and Gamma nail internal fixation in the treatment of intertrochanteric fracture of femur.Methods:Sixty patients with intertrochanteric fracture of the femur admitted to Guangzhou Nansha District Second People's Hospital from May 2021 to May 2023 were selected for the study,and were randomly divided into the PFNA group and the Gamma group,with thirty patients in each group.Patients in the Gamma group were internally fixed with Gamma nails,and patients in the PFNA group were internally fixed with PFNA.The perioperative indexes(operation time,intraoperative bleeding,weight-bearing time,fracture healing time),excellent efficacy rate,Harris score of hip function and postoperative complications were compared between the two groups.Results:The operation time and fracture healing time of the PFNA group were shorter than those of the Gamma group,the intraoperative bleeding was less than that of the Gamma group,and the weight-bearing time was longer than that of the Gamma group,and the differences were statistically significant(P<0.001);The excellent rate of efficacy in the PFNA group was higher than that in the Gamma group,and the difference was statistically significant(P=0.044);After treatment,Harris scores increased in both groups,and the PFNA group was higher than the Gamma group,with a statistically significant difference(P<0.001);The incidence of postoperative com⁃plications was lower in the PFNA group than in the Gamma group,and the difference was statistically significant(P=0.044).Con⁃clusion:The efficacy of PFNA internal fixation for intertrochanteric fracture of the femur is better than that of Gamma nail internal fixation,and the postoperative complications are slight,the recovery of hip function is good,and it is beneficial to shorten the heal⁃ing time of the fracture,which is worthy of popularization and in-depth investigation.
