医学营养治疗结合平胃散加减对孕前高体重指数痰湿质孕妇体重增长及妊娠结局的影响

摘　　要：目的探讨医学营养治疗(MNT)结合平胃散加减对孕前高体重指数(BMI)痰湿质孕妇体重增长及妊娠结局的影响。方法选取2021年1月至2022年12月在佛山市南海区妇幼保健院产科定期产检并分娩的116例孕前BMI>24 kg/m~2痰湿质孕妇,按不同的治疗方式分为对照组(29例)、MNT组(28例)、结合组(59例)。对照组接受常规产前检查及一般健康教育,MNT组在对照组的基础上增加MNT,结合组在MNT组的基础上增加平胃散加减治疗,比较各组孕期体重增长情况、妊娠并发症发生情况、分娩方式及新生儿体重。结果各组孕期增重情况比较,差异有统计学意义(P<0.05),结合组孕期增重适宜率高于对照组、MNT组(P<0.05);各组妊娠高血压、妊娠糖尿病发生率比较,差异有统计学意义(P<0.05);结合组、MNT组阴道分娩率高于对照组(P<0.05);各组新生儿出生体重比较,差异无统计学意义(P>0.05)。结论MNT结合平胃散加减应用于孕前高BMI痰湿质孕妇,可以提高孕期增重适宜率、自然分娩率,降低妊娠并发症发生率。Objective To investigate the effect of medical nutrition therapy(MNT)combined with modified Pingwei Powder on weight gain and pregnancy outcome in pregnant women with pre-pregnancy high body mass index(BMI)and phlegm-dampness constitution.Methods A total of 116 pregnant women with pre-pregnancy BMI>24 kg/m2 and phlegm-dampness constitution who had regular prenatal examination and delivered in Department of Obstetrics,Foshan Nanhai District Women and Children Hospital from January 2021 to December 2022 were selected.According to different treatment methods,they were divided into control group(29 cases),MNT group(28 cases)and combined group(59 cases).The control group received routine prenatal examination and general health education,the MNT group was added MNT on the basis of the control group,and the combined group was added modified Pingwei Powder on the basis of the MNT group.The weight gain during pregnancy,pregnancy complications,delivery mode and neonatal weight were compared among the groups.Results There was a significant difference in weight gain during pregnancy among the groups(P<0.05),and the appropriate rate of weight gain during pregnancy in the combined group was higher than that in the control group and the MNT group(P<0.05).There were statistically significant differences in the incidence rates of gestational hypertension and gestational diabetes mellitus in each group(P<0.05).The vaginal delivery rate in the combined group and MNT group was higher than that in the control group(P<0.05).There was no significant difference in birth weight among all groups(P>0.05).Conclusion The application of MNT combined with modified Pingwei powder in pregnant women with pre-pregnancy high BMI and phlegm-dampness constitution can improve the appropriate rate of weight gain during pregnancy,the rate of natural delivery,and reduce the incidence of pregnancy complications.
