卡前列素氨丁三醇联合宫腔填纱治疗前置胎盘产后出血的效果

摘　　要：目的:探讨卡前列素氨丁三醇联合宫腔填纱治疗前置胎盘产后出血的效果。方法:选取2021年4月—2023年4月滕州市中心人民医院收治的80例前置胎盘产后出血患者作为研究对象,随机分为对照组与观察组,各40例。对照组采用缩宫素联合宫腔填纱治疗,观察组采用卡前列素氨丁三醇联合宫腔填纱治疗。比较两组出血量、不良反应发生情况、治疗效果、血压和心率。结果:观察组产后2 h和产后24 h出血量均少于对照组,停止出血时间短于对照组,差异有统计学意义(P<0.001)。观察组不良反应总发生率低于对照组,差异有统计学意义(P=0.038)。观察组治疗总有效率高于对照组,差异有统计学意义(P=0.023)。治疗前,两组舒张压、收缩压、心率比较,差异无统计学意义(P>0.05);治疗后,对照组舒张压、收缩压、心率均高于治疗前,观察组舒张压、收缩压、心率均低于对照组,差异有统计学意义(P<0.05)。结论:卡前列素氨丁三醇联合宫腔填纱治疗前置胎盘产后出血的效果理想,能减少患者出血量,缩短出血时间,减少不良反应,平稳血压和心率。Objective:To investigate the therapeutic effect of carboprost tromethamine combined with intrauterine gauze tamponade in the treatment of postpartum hemorrhage with placenta praevia.Methods:80 cases of postpartum hemorrhage with placenta praevia admitted to Tengzhou Central People's Hospital from April 2021 to April 2023 were selected as the study subjects,and were randomly divided into the control group and the observation group,40 cases each.The control group was treated with hysterotonin combined with intrauterine gauze tamponade,and the observation group was treated with carboprost tromethamine combined with intrauterine gauze tamponade.Bleeding volume,occurrence of adverse reactions,therapeutic effect,blood pressure and heart rate were compared between the two groups.Results:The bleeding volume in the observation group was less than that in the control group 2 hours and 24 hours after delivery,and the time to stop bleeding was shorter than that in the control group,with statistically significant differences(P<0.001).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.038).The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.023).Before treatment,the diastolic blood pressure,systolic blood pressure and heart rate of the two groups were compared,the difference was not statistically significant(P>0.05);after treatment,the diastolic blood pressure,systolic blood pressure and heart rate of the control group were higher than those before treatment,and the diastolic blood pressure,systolic blood pressure and heart rate of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion:Carboprost aminoglutethrin combined with intrauterine gauze tamponade is ideal for the treatment of postpartum hemorrhage with placenta praevia,which can re
