二维超声联合经阴道三维超声诊断子宫瘢痕妊娠的临床价值

摘　　要：目的:探讨二维超声联合经阴道三维超声对子宫瘢痕妊娠的诊断价值。方法:选取2020年6月—2023年6月扬州市江都妇幼保健院收治的疑似子宫瘢痕妊娠者48例为研究对象,均接受二维超声、经阴道三维超声检查。以手术结果或病理结果作为诊断“金标准”,比较二维超声检查、二维联合阴道三维超声检查子宫瘢痕妊娠的结果与诊断效能。结果:联合检查诊断符合率与金标准比较,差异无统计学意义(P>0.05);联合检查诊断符合率高于二维超声检查,差异有统计学意义(P=0.021);联合检查对子宫瘢痕妊娠的灵敏度、准确度高于二维超声检查,差异有统计学意义(P<0.05)。结论:二维超声联合经阴道三维超声检查对于诊断子宫瘢痕妊娠的灵敏度、准确率高,可获得丰富的影像特征信息,可为疾病的早期治疗干预提供帮助。Objective:To explore the diagnostic value of two-dimensional ultrasound combined with transvaginal threedimensional ultrasound for uterine scar pregnancy.Methods:A total of 48 patients with suspected uterine scar pregnancy admitted to Yangzhou Jiangdu Maternity and Child Health Hospital from June 2020 to June 2023 were selected as the study subjects.All patients received two-dimensional ultrasonography and vaginal three-dimensional ultrasonography.The results of surgery or pathology were taken as the diagnostic gold standard,and the diagnostic efficiency of two-dimensional ultrasonography and twodimensional combined with transvaginal three-dimensional ultrasonography for uterine scar pregnancy was analyzed.Results:Among 48 patients with suspected uterine scar pregnancy,35 cases(72.92%)were confirmed positive by gold standard,including 30 cases of gestation sac type and 5 cases of mass type.There was no significant difference in the diagnostic coincidence rate between combined examination and gold standard(P>0.05);The diagnostic coincidence rate of combined examination was higher than that of two-dimensional ultrasonography,and the difference was statistically significant(P=0.021);The sensitivity and accuracy of combined examination for uterine scar pregnancy were higher than those of two-dimensional ultrasonography,and the difference was statistically significant(P<0.05).Conclusion:Two-dimensional ultrasonography combined with transvaginal threedimensional ultrasonography has high accuracy in the diagnosis of uterine scar pregnancy,and can obtain rich image characteristics information,which can provide help for early treatment intervention of the disease.
