盆底肌群训练联合口服他达拉非在骨盆骨折后ED患者中应用

摘　　要：目的 探究骨盆骨折伴尿道损伤所致勃起功能障碍患者骨折复位固定术后盆底肌群康复训练联合口服小剂量他达拉非的临床疗效。方法 选取2018年9月至2022年9月我院康复科收治的110例骨盆骨折伴尿道损伤所致勃起功能障碍的男性患者为研究对象,所有患者均接受骨折复位固定术治疗。按照患者术后接受的干预措施不同,分为观察组和对照组各55例,其中对照组术后接受口服小剂量他达拉非治疗,观察组在对照组的基础上采取盆底肌群康复训练治疗。两组患者连续干预1个月后进行疗效观察,对比两组的IIEF-5评分和NPT情况。结果 干预前两组患者的各项评分对比无显著差异(P>0.05);干预后两组患者的IIEF-5评分均得到了显著提升,其中观察组显著高于对照组(P<0.05);干预后,两组患者的NPT指标中勃起硬度、勃起维持时间、次数均得到了显著改善,其中观察组较对照组改善情况更好(P<0.05)。两组在治疗期间均未出现严重的不良反应(P>0.05)。结论 骨盆骨折伴尿道损伤所致器质性勃起功能障碍患者术后采用小剂量他达拉非联合盆底肌群康复训练能有效改善患者的勃起功能,不良反应可控,远期疗效有待于进一步观察。Objective To explore the clinical efficacy of oral low⁃dose tadalafil combined with pelvic floor muscle rehabilitation training in the patients with erectile dysfunction caused by pelvic fracture and urethral injury after bone repositioning and fixation.Methods From September 2018 to September 2022,110 male patients with erectile dysfunction caused by pelvic fracture and urethral injury admitted to our hospital were selected as the research subjects.All patients received bone repositioning and fixation surgery,and were divided into an observation group and a control group of 55 patients according to the intervention measures they received after surgery.The control group received oral low⁃dose tadalafil treatment after surgery.On the basis of the control group,the observation group received pelvic floor muscle group rehabilitation training treatment.After continuous intervention for 1 month,the curative effects of the two groups were evaluated,and the IIEF⁃5 score and NPT were comparatively analyzed between the two groups.Results No significant differences were found in the scores of IIEF⁃5 between the two groups before intervention(P>0.05),whereas IIEF⁃5 scores of the two groups of patients were all significantly improved after intervention,moreover IIEF⁃5 scores in the observation group increased more significantly(P<0.05).After intervention,the intensity,duration and frequency of erection in NPT indicators in the two groups were obviously improved,moreover the observation group showed better improvement(P<0.05).No serious adverse reactions in two groups during the treatment were observed(P>0.05).Conclusion For the patients with organic erectile dysfunction caused by pelvic fractures and urethral injury after low⁃dose tadalafil combined with pelvic floor muscle rehabilitation training can effectively improve the erectile function of patients.The side effects are controllable,and the long⁃term efficacy needs further observation.
