摘　　要：目的:分析老年人多重慢性病的现状及其对医疗费用的影响,提出预防和管理老年人多重慢性病的建议。方法:利用CLHLS数据,统计分析老年人多重慢性病情况,运用Tobit回归模型,分析多重慢性病对医疗费用的影响。结果:7483位受访老年人中,患慢性病的有5142人(68.7%),患2种及以上慢性病的有2852人(38.1%);老年人患慢性病种数与医疗费用呈正相关,每增加1种慢性病,门诊总费用增加947.46元、门诊自付费用增加477.70元、住院总费用增加1192.43元、住院自付费用增加607.44元。结论:我国老年人多重慢性病患病率较高,对此应加强老年人健康宣教,落实分级诊疗制度,促进医防融合,提高慢性病医疗保障水平。Objective To analyze the prevalence of multiple comorbidities in the elderly and their impact on medical expenses,and to propose suggestions for preventing and managing multiple comorbidities in the elderly.Methods Using CLHLS data,the prevalence of multiple comorbidities in the elderly was analyzed.The Tobit regression model was used to analyze the impact of multiple comorbidities on medical expenses.Results Among the 7483 elderly respondents,5142(68.7%)had chronic diseases,and 2852(38.1%)had two or more chronic diseases.The number of chronic diseases in the elderly was positively correlated with medical expenses.For each additional chronic disease,the total outpatient expenses increased by 947.46 yuan,the outpatient out of pocket expenses increased by 477.70 yuan,the total hospitalization expenses increased by 1192.43 yuan,and the hospitalization out of pocket expenses increased by 607.44 yuan.Conclusion The prevalence of multiple comorbidities among the elderly in China is relatively high.It is necessary to strengthen health education for the elderly,implement a tiered diagnosis and treatment system,promote the integration of medical and preventive measures,and improve the level of medical security for chronic diseases.
