Multiloc髓内钉治疗老年骨质疏松性肱骨近端骨折的临床疗效

摘　　要：目的探讨Multiloc髓内钉对老年骨质疏松性肱骨近端骨折的临床治疗效果。方法选取2017年3月-2020年6月北京市垂杨柳医院收治的肱骨近端骨折合并骨质疏松老年患者47例,依据手术方式分为观察组(24例)与对照组(23例),对照组行Phlios钢板内固定治疗,观察组行Multiloc髓内钉内固定治疗。比较两组手术指标、手术前后视觉模拟疼痛量表评分(VAS)、欧洲肩关节协会评分(Constant)及并发症情况。结果观察组手术时间、住院时间、术中出血量低于对照组,差异有统计学意义(P<0.05);末次随访时,观察组VAS评分低于对照组,肩关节功能Constant评分高于对照组,差异有统计学意义(P<0.05);观察组术后并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论Multiloc髓内钉固定治疗老年骨质疏松性肱骨近端骨折可有效改善患者肩关节功能,疼痛轻、并发症少,有利于肩关节活动恢复。Objective To investigate the clinical treatment effect of Multiloc intramedullary nail on osteoporotic proximal humeral fracture in the elderly patients.Methods A total of 47 elderly patients with osteoporotic proximal humeral fracture admitted to Beijing Chuiyangliu Hospital from March 2017 to June 2020 were selected and divided into observation group(24 patients)and control group(23 patients)according to the operation method.The control group was treated with Phlios plate internal fixation,and the observation group was treated with Multiloc intramedullary nail internal fixation.The surgical indicators,Visual Analogue Pain Scale(VAS)score,European Shoulder Association score(Constant)and complications before and after surgery were compared between the two groups.Results The operation time,hospitalization time and intraoperative blood loss in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).At the last follow-up,the VAS score of the observation group was lower than that of the control group,and the Constant score of shoulder joint function was higher than that of the control group,the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Multiloc intramedullary nail fixation can effectively improve the shoulder joint function for the treatment of osteoporotic proximal humeral fractures in the elderly,with less pain and fewer complications,which is conducive to the recovery of shoulder joint activity.
