摘 要:目的 探讨动态脑电图(AEEG)与头颅磁共振成像(MRI)在小儿癫痫合并精神障碍及病灶定位中的诊断价值,为临床提供参考。方法 回顾性分析2020年1月至2022年12月湖北医药学院附属襄阳市第一人民医院收治的210例疑似癫痫合并精神障碍患儿的临床资料。入院后均进行AEEG和头颅MRI检查,以手术病理诊断结果为金标准,比较AEEG、头颅MRI对癫痫合并精神障碍患儿的诊断效能,并分析AEEG、头颅MRI对癫痫合并精神障碍患儿病灶的定位结果。结果 手术病理诊断阳性128例,阴性82例;AEEG诊断阳性126例,阴性84例;头颅MRI诊断阳性124例,阴性86例。一致性检验结果显示,AEEG、头颅MRI诊断癫痫合并精神障碍的Kappa值分别为0.940、0.722(P <0.05)。128例确诊癫痫合并精神障碍患儿的病灶分布于颞叶区64例、枕叶区13例、额叶区44例及顶叶区7例。AEEG对癫痫合并精神障碍患儿颞叶区、枕叶区、额叶区病灶的检出率高于头颅MRI(P<0.05)。结论 AEEG在小儿癫痫合并精神障碍及病灶定位中的诊断价值优于头颅MRI,值得临床应用。Objective To explore the diagnostic value of ambulatory electroencephalography(AEEG)and cranial resonance imaging(MRI)in pediatric epilepsy complicated with mental disorders and lesion localization,to provide reference for clinical practice.Methods The clinical data of 210 children with suspected epilepsy complicated with mental disorders admitted to the Xiangyang No.1 People’s Hospital Affiliated to Hubei University of Medicine from January 2020 to December 2022 were retrospectively analyzed.After admission,AEEG and cranial MRI were performed in all children,and the surgical pathological diagnosis was the gold standard.The diagnostic efficiency of AEEG and cranial MRI in epilepsy complicated with mental disorders was compared,and the localization results of AEEG and cranial MRI in epilepsy complicated with mental disorders were analyzed.Results The pathological diagnosis was positive in 128 cases and negative in 82 cases;AEEG diagnosis was positive in 126 cases and negative in 84 cases;Cranial MRI diagnosis was positive in 124 cases and negative in 86 cases.The results of consistency analysis showed that the Kappa value of AEEG and cranial MRI diagnosis of children with epilepsy complicated with mental disorders was 0.940 and 0.722,respectively(P<0.05).Among the 128 cpediatric epilepsy complicated with mental disorders,64 cases were located in the temporal lobe,13 in the occipital lobe,44 in the frontal lobe and 7 in the parietal lobe.The detection rate of AEEG for lesions in the temporal,occipital,and frontal lobes of pediatric epilepsy complicated with mental disorders was higher than that of cranial MRI(P<0.05).Conclusion The diagnostic value of AEEG in pediatric epilepsy complicated with mental disorders and lesion localization is superior to that of cranial MRI,and it is worthy of clinical application.
