彩色多普勒超声诊断乳腺良恶性肿块的临床价值

摘　　要：目的:分析彩色多普勒超声诊断乳腺良恶性肿块的临床价值。方法:回顾性分析2022年1月—12月济宁市金乡县人民医院收治的乳腺疾病患者120例(132个乳腺肿块)的临床资料,患者均接受彩色多普勒超声诊断。以病理诊断结果为金标准,分析彩色多普勒超声诊断效能,观察良恶性肿块影像学特征。结果:彩色多普勒超声诊断灵敏度93.98%、特异度95.92%、准确率94.70%,Kappa值=0.888,与金标准一致性较强。乳腺恶性肿块组的血流分级主要为Ⅱ级38例、Ⅲ级45例;乳腺良性肿块组血流分级主要为0级27例,Ⅰ级22例。乳腺恶性肿块与良性肿块在血流分级比较差异有统计学意义(P <0.05)。良恶性乳腺肿块在边界、形状、有无血流、血流参数方面差异有统计学意义(P <0.05),良性肿块呈现为清晰、规则的边界,形状规则,表面光滑,血流较少。恶性肿块边界模糊、不规则或毛刺状,超声图像显示异常血管结构和血流分布,血流速度高于正常乳腺组织或良性肿块。结论:彩色多普勒超声在乳腺良恶性肿块鉴别诊断中具有潜力,并可提供有关血流分布和图像特征的信息,为临床上乳腺癌的早期诊断和治疗提供详细的参考依据。Objective Analyze the clinical value of color Doppler ultrasound in diagnosing benign and malignant breast masses.Methods Clinical data of 120 patients with breast diseases(132 breast masses)admitted to Jinxiang County People's Hospital of Jining City from January to December 2022 were retrospectively analyzed.All patients were diagnosed by color Doppler ultrasound.According to the gold standard of pathological diagnosis,the diagnostic efficiency of color Doppler ultrasound was analyzed,and the imaging features of benign and malignant masses were observed.Results The diagnostic sensitivity of color Doppler ultrasound was 93.98%,the specificity 95.92%,the accuracy 94.70%,Kappa value=0.888,and the consistency with gold standard was strong.In the breast malignant mass group,38 cases were gradeⅡand 45 cases were gradeⅢ.In the benign breast mass group,the main blood flow grading was grade 0 in 27 cases and gradeⅠin 22 cases.There were significant differences in blood flow grading between malignant and benign breast masses(P<0.05).There were significant differences in boundary,shape,presence or absence of blood flow and blood flow parameters between benign and malignant breast masses(P<0.05).Benign tumors showed clear and regular boundary,regular shape,smooth surface and less blood flow.Malignant masses have blurred,irregular,or burring borders,and ultrasound images show abnormal vascular structure and blood flow distribution with higher blood flow rates than normal breast tissue or benign masses.Conclusion Color Doppler ultrasound has potential in the differential diagnosis of benign and malignant breast masses,and can provide information about blood flow distribution and image characteristics,which can provide detailed reference for early diagnosis and treatment of breast cancer clinically.
