摘 要:目的:研究全真一气汤对肾不纳气证慢性阻塞性肺疾病大鼠横膈肌细胞形态和横膈肌收缩功能的影响,探讨该汤药的治疗作用及横膈在肾不纳气证形成中的意义。方法:70只SD雄性大鼠随机分为空白组、模型组、中药实验组(予全真一气汤)、中药对照组(予六君子汤)和西药对照组,苏木精-伊红染色观察大鼠肺和横膈组织病理形态,透射电子显微镜下观察大鼠横膈组织超微结构;采用MD3000生物信号采集处理系统检测大鼠横膈组织肌张力。结果:苏木精-伊红染色,模型组和西药对照组,两中药组气道上皮细胞变形坏死减轻,炎症细胞浸润缓解,肺泡融合减少,其中中药实验组优于中药对照组。两中药组横膈肌肉组织损伤减轻,其中中药实验组轻于中药对照组。透射电子显微镜下,中药实验组大鼠横膈组织肌细胞损伤明显减轻,而中药对照组大鼠横膈组织肌细胞萎缩,排列模糊紊乱,超微结构破坏情况与西药对照组、模型组相似。体外横膈肌收缩力检测,与空白组比较,肾不纳气证慢性阻塞性肺疾病各组大鼠横膈组织肌张力明显降低,弹性变差(P <0.01),中药实验组相比较模型组和西药对照组,肌张力升高,弹性改善,差异有统计学意义(P <0.05),中药对照组与模型组、西药对照组比较则差异无统计学意义(P> 0.05)。结论:辨证论治使用中药方剂可以更好地改善慢性阻塞性肺疾病引起的肺和横膈组织损伤,膈肌功能与中医肾不纳气证形成有关。Objective:To study the effects of the Quanzhen Yiqi decoction(全真一气汤)on the morphology of diaphragm cells and the contractile function of diaphragm muscle in rats with chronic obstructive pulmonary disease with the kidney dysfunction in Qi(气)holding syndrome,and to explore the therapeutic effect of decoction and the significance of diaphragm in the formation of the kidney dysfunction in Qi holding syndrome.Methods:Seventy male SD rats were randomly divided into the blank group,the model group,the TCM experimental group(given the Quanzhen Yiqi decoction),the TCM control group given the Liujunzi decoction(六君子汤)and the western medicine control group.At the end of the experiment,the pathological morphology of lung and diaphragm of rats was observed by light microscope after hematoxylin-eosin staining,and the ultrastructure of diaphragm of rats was observed by transmission electron microscope.MD3000 biosignal acquisition and processing system was used to detect the muscle tension in the diaphragm of rats.Results:Hematoxylin-eosin staining showed that the deformation and necrosis of airway epithelial cells in TCM groups were alleviated,the infiltration of inflammatory cells was alleviated,and the alveolar fusion was reduced.Those in the TCM experimental group were slightly better than the TCM control group.The diaphragm injury in the TCM groups were alleviated,and that in the TCM experimental group was lighter than that in the TCM control group.Under transmission electron microscope,the damage of diaphragm cells in the experimental group was reduced obviously,while the diaphragm fibers in the TCM control group were atrophic,blurred and disordered,and ultrastructural damage was similar to that in the western medicine control group and the model group.Compared with the blank group,muscular tension of the diaphragm of COPD rats was significantly decreased and the elasticity became worse(P<0.01).Compared with the model group and the western medicine control group,muscular tension of the diaphragm was inc
