高强度聚焦超声治疗产后胎盘植入的临床疗效

摘　　要：目的分析高强度聚焦超声(HIFU)治疗产后胎盘植入的临床疗效。方法选取2018年1月至2021年11月贵州盘江煤电集团有限责任公司医院收治的42例产后胎盘植入患者作为研究对象,依据随机数表法分为对照组与观察组,每组21例。对照组予以介入治疗,观察组予以HIFU治疗,比较两组手术时间、β-人绒毛膜促性腺激素(β-HCG)降至正常时间、住院时间、恢复月经时间、子宫复旧时间、术后清宫率、胎盘自然排出率和并发症发生情况。结果观察组手术时间长于对照组,β-HCG降至正常时间、恢复月经时间、子宫复旧时间及住院时间均短于对照组,术后清宫率低于对照组,胎盘自然排出率高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论HIFU治疗产后胎盘植入患者疗效确切,能促进患者术后月经、子宫和β-HCG恢复,缩短住院时间,降低清宫率,提升胎盘自然排出率,减少并发症的发生,值得临床推广应用。Objective To analyze the clinical efficacy of high-intensity focused ultrasound(HIFU)in the treatment of postpartum placental im-plantation.Methods 42 patients with postpartum placental implantation admitted to Guizhou Panjiang Coal and Power Group Co.,Ltd.Hospital from January 2018 to November 2021 were selected as the study subjects,they were divided into the uncontrolled group and the observation group according to the random number table method,with 21 cases in each group.The control group received intervention treatment,while the observation group received HIFU treatment,the surgical time andβ-human chorionic gonadotropin(β-HCG)reduction to normal time,hospital stay,menstrual recovery time,uterine involution time,postoperative curettage rate,natural placental excretion rate,and incidence of complications were compared between the two groups.Results The surgical time in the observation group was longer than that in the control group,β-HCG reduction to normal time,hospital stay,menstrual recovery time,uterine involution time were all shorter than those in the control group,the postoperative curettage rate was lower than that in the control group,and the natural placental excretion rate was higher than that in the control group,the differences were statis-tically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion HIFU is effective in the treatment of postpartum placental implantation,can promote recovery of menstruation,uterus andβ-HCG,shorten the hospital stay,reduced postoperative curettage rate,improved natural placental excretion rate,and re-duced incidence of complications,which is worthy of clinical promotion and application.
