摘  要:目的:探讨自动乳腺全容积成像系统(ABVS)冠状面联合超声弹性成像(UE)技术鉴别乳腺影像报告和数据超声系统(BI-RADS-US)4类病灶良恶性的诊断价值。方法:收集2019年1月至2022年12月常规手持超声(HHUS)诊断为BI-RADS-US 4类并接受诊治的276例乳腺疾病患者(共294个病灶)。所有患者术前均行ABVS及UE两种检查,以病理结果为金标准,分别构建预测模型。结果:HHUS、UE、ABVS冠状面以及ABVS冠状面联合UE诊断乳腺BI-RADS-US 4类病灶的曲线下面积(AUC)分别为0.845、0.837、0.873、0.902,ABVS冠状面联合UE的AUC与HHUS、UE、ABVS比较差异均有统计学意义(Z值分别为3.164、2.056、2.087,均P<0.05)。结论:ABVS冠状面联合UE可以提高BI-RADS-US 4类乳腺病灶的诊断效能,可以有效地降低良性病灶穿刺活检率并提高恶性病灶检出率,有着较高的临床应用价值。Objective:To explore the diagnostic value of automated breast volume scanner(ABVS)coronal combined with ultrasound elastography(UE)in differentiating benign and malignant lesions of BI-RADS-US category 4.Methods:276 breast disease patients(294 lesions)diagnosed as BI-RADS 4 by conventional handheld ultrasound(HHUS)from January 2019 to December 2022 were selected.All patients were examined by ABVS and UE before operation.Based on the pathological results as the gold standard and construct the prediction model separately.Results:The area under the curve(AUC)for the diagnosis of BI-RADS-US category 4 lesions by HHUS,UE,ABVS coronal and ABVS coronal combined with UE were 0.845,0.837,0.873,0.902,respectively.The difference in AUC of ABVS coronal combined with UE were statistically significant when compaved with HHUS,UE,and ABVS(Z=3.164,2.056,2.087,P<0.05).Conclusion:ABVS coronal plane combined with UE can improve the diagnostic efficiency of BI-RADS-US 4 breast lesions,effectively reduce the rate of puncture biopsy of benign lesions and improve the detection rate of malignant lesions,which has high clinical application value.
