同步水降温电凝止血法与缝合止血法用于老年腹腔镜卵巢囊肿剥除术止血效果对比及对性激素水平的影响

摘　　要：目的对比同步水降温电凝止血法与缝合止血法用于老年腹腔镜卵巢囊肿剥除术止血效果及对性激素水平的影响。方法回顾性分析泗县人民医院2020年1月~2022年12月收治的60例行腹腔镜卵巢囊肿剥除术老年患者,按照术中止血方法分为对照组(缝合止血法,n=30)及观察组(同步水降温电凝止血法,n=30),记录两组患者术后相关指标,并对比两组患者应激反应指标、性激素水平及并发症发生情况。结果两组患者囊肿直径、手术时间、术后排气时间对比差异无统计学意义(P>0.05);观察组术中失血量少于对照组、住院时间短于对照组,差异有统计学意义(P<0.05)。观察组患者术后去甲肾上腺素(NE)、促肾上腺皮质激素(ACTH)、皮质醇(Cor)水平均低于对照组,差异有统计学意义(P<0.05);两组患者术后1个月雌二醇(E2)、促黄体生成素(LH)及促卵泡刺激素(FSH)水平对比差异无统计学意义(P>0.05);两组患者术后并发症对比差异无统计学意义(P>0.05)。结论在老年腹腔镜卵巢囊肿剥除术中应用同步水降温电凝止血法相较于缝合止血法止血效果更佳,更利于患者术后恢复,还能减轻患者手术应激,且同步水降温电凝止血法对患者性激素水平影响程度与缝合止血法接近,具有一定安全性。Objective To compare the hemostatic effect of synchronous water cooling electrocoagulation hemostasis method and suture hemostasis method on elderly patients with laparoscopic ovarian cystectomy and the influence on levels of sex hormones.Methods The clinical data of 60 elderly patients undergoing laparoscopic ovarian cystectomy in the hospital from January 2020 to December 2022 were retrospectively analyzed.According to the intraoperative hemostasis method,the patients were divided into control group(suture hemostasis method,n=30)and observation group(simultaneous water cooling electrocoagulation hemostasis method,n=30).The postoperative related indicators were recorded in the two groups of patients,and the stress response indicators,sex hormones levels and occurrence of complications were compared between the two groups of patients.Results There were no statistically significant differences in cyst diameter,surgical time and postoperative exhaust time between the two groups(P>0.05).The observation group had less intraoperative blood loss and shorter hospital stay than the control group,with statistically significant differences(P>0.05).The levels of norepinephrine(NE),adrenocorticotropic hormone(ACTH)and cortisol(Cor)in observation group after surgery were lower than those in control group(P>0.05).The differences in the levels of estradiol(E2),luteinizing hormone(LH)and follicle stimulating hormone(FSH)were not statistically significant between the two groups at 1 month after surgery(P>0.05).There were no statistical differences in the postoperative complications between the two groups(P>0.05).Conclusion Compared with suture hemostasis method,the application of synchronous water cooling electrocoagulation hemostasis method during laparoscopic ovarian cystectomy of the elderly has better hemostatic effect,and it is more conducive to the postoperative recovery of patients,and can better relieve the surgical stress of patients.The influence of synchronous water cooling electrocoagulation hemostasis method on the s
