乙肝病毒感染相关肝细胞癌组织中转化因子-β1/Smad2信号通路表达及其与临床病理特征、预后相关性

摘  要:背景肝癌的发生和发展受多种经典信号通路的调控影响,癌细胞的生长或是被抑制或是被促进.其中,转化生长因子-β1(transforming growth factor-β1,TGF-β1)/Smad2信号通路在癌细胞的发展研究中较为经典.目的探讨TGF-β1/Smad2信号通路在肝细胞癌组织中表达及其与乙肝病毒(hepatitis B virus,HBV)感染相关性.方法选取2016-02/2019-03本院经手术切除后病理诊断明确的肝细胞癌患者65例为研究对象,依据是否发生HBV感染分为感染者37例、非感染者28例.随访3年后失访8例,根据3年内生存情况肝细胞癌患者分为生存者36例、死亡者21例.采用蛋白免疫印迹法检测肝细胞癌组织、癌旁组织中TGF-β1、Smad2表达量.分析HBV感染者、非感染者肝细胞癌组织中TGF-β1、Smad2蛋白表达量与临床病理特征相关性,及其与生存率相关性.Cox回归分析预后影响因素.结果HBV感染者、非感染者肝细胞癌组织中TGF-β1、Smad2表达量高于癌旁组织,且HBV感染者高于非感染者(P<0.05);TGF-β1、Smad2表达量与分化程度、卫星病灶、肝硬化、TNM分期、淋巴结转移有关(P<0.05);死亡者肝细胞癌组织中TGF-β1、Smad2表达量高于生存者(P<0.05);TGF-β1高表达者3年内生存率48.28%低于低表达者78.57%(P=0.003);Smad2高表达者3年内生存率50.00%低于低表达者77.78%(P=0.012);TNM分期、淋巴结转移、TGF-β1表达量、Smad2表达量为3年内预后不良的独立危险因素(P<0.05).结论HBV感染肝细胞癌组织中TGF-β1、Smad2表达量升高,且与临床病理特征、预后不良有关.BACKGROUND The occurrence and development of liver cancer are affected by many classical signaling pathways,and as a result,the growth of cancer cells is either inhibited or promoted.The transforming growth factor-β1(TGF-β1)/Smad2 signaling pathway is often involved in the development of cancer.AIM To investigate the expression of TGF-β1 and Smad2 in hepatocellular carcinoma tissues and its correlation with hepatitis B virus(HBV)infection.METHODS Sixty-five patients with hepatocellular carcinoma diagnosed by pathology after surgical resection at our hospital from February 2016 to March 2019 were selected for the study,and they were divided into 37 HBV-infected cases and 28 non-infected cases based on whether HBV infection occurred.Eight cases were lost after 3 years of follow-up,and the remaining patients with hepatocellular carcinoma were divided into two subgroups based on their survival status:36 cases in survival group and 21 cases in death group.The expression of TGF-β1 and Smad2 in liver cancer tissues and paraneoplastic tissues was detected by protein immunoblotting(Western blot).The correlation of TGF-β1 and Smad2 protein expression with clinicopathological characteristics and survival rate was analyzed.Cox regression analysis of prognostic factors was performed.RESULTS The expression of TGF-β1 and Smad2 in hepatocellular carcinoma tissues of both HBV-infected and non-infected patients was higher than that in paracancerous tissues,and their expression in HBV-infected patients was higher than that of non-infected patients(P<0.05).The expression of TGF-β1 and Smad2 was associated with the degree of differentiation,satellite lesions,cirrhosis,TNM stage,and lymph node metastasis(P<0.05).TGF-β1 and Smad2 expression was higher in dead patients than in survivors(P<0.05).The survival rate within 3 years was lower in patients with high TGF-β1 expression(48.28%)than in those with low expression(78.57%;P=0.003).The survival rate within 3 years was also lower inpatients with high Smad2 expression(50.00%)than
