开窍药治疗缺血性脑卒中的作用机制研究进展

摘　　要：有关开窍药治疗缺血性脑卒中的研究较多但缺乏作用机制的深入分析以及规律性总结。本文综述了近5年来开窍药治疗缺血性脑卒中的研究进展,结果发现开窍药现代制剂(醒脑静注射液、清开灵注射液),经典方剂(安宫牛黄丸),单药(麝香、石菖蒲、苏合香、安息香、冰片、牛黄等及其有效成分)对缺血性脑卒中具有积极的治疗作用。开窍药治疗缺血性脑卒中的机制主要与双向调节血脑屏障、调节氧化应激反应、抑制谷氨酸兴奋性毒性、抑制炎症反应和细胞凋亡、调节Ca^(2+)超载、干预细胞自噬有关。In spite of many studies on the treatment of ischemic stroke with Kaiqiao(开窃)drugs,there is a lack of in-depth analysis and regular summary of the mechanism.This paper reviewed the research progress in Kaiqiao drugs against ischemic stroke in the past five years.The results showed that modern preparations(Xingnaojing(醒脑静)Injection and Qingkailing(清开灵)Injection),classical prescriptions(Angong Niuhuang(安宫牛黄)Pills),and single drugs(Shexiang(麝香),Shichangpu(石菖蒲),Suhexiang(苏合香),Anxixiang(安息香),Bingpian(冰片),and Niuhuang(牛黄),as well as their active components)had a positive therapeutic effect on ischemic stroke.The mechanism of Kaiqiao drugs in the treatment of ischemic stroke was mainly related to the bi-directional regulation of the blood-brain barrier,regulation of oxidative stress,inhibition of glutamate excitotoxicity,inhibition of inflammatory reaction and apoptosis,regulation of Ca^(2+)over load,and intervention of autophagy.
