摘 要:目的:应用根因分析法(root cause analysis, RCA)对药库管理中出现的1例临界差错进行分析,以提高药库药品管理水平。方法:成立RCA小组,对事件进行梳理和复盘,并利用头脑风暴会议、鱼骨图分析法、“5why”分析法等分析近端原因和根本原因,并制定改进方案。结果:根据根本原因分析结果针对性地制定了改善,从工作制度、信息流程、业务培训、有效沟通等4个方面进行改进。结论:通过积极、有效地分析药品价格错误发生的根本原因,发现了日常管理中许多不足之处并实施有效的干预措施,提高了药库管理水平。OBJECTIVE To apply root cause analysis(RCA)to analyze a critical error in drug warehouse management,in order to improve the level of drug warehouse management.METHODS Establish an RCA team to sort out and review events,and use brainstorming sessions,fishbone analysis,and 5why analysis to analyze proximal and root causes,and develop improvement plans.RESULTS According to the results of RCA,targeted improvement actions have been formulated.Improve from four aspects:work system,information process,business training,and effective communication.CONCLUSION By actively and effectively analyzing the root causes of drug price errors,many shortcomings in daily management have been identified and effective intervention measures have been implemented,improving the level of drug warehouse management.
