摘 要:目的 分析升阳益胃汤加减治疗神经重症患者营养性腹泻的临床疗效。方法 80例神经重症营养性腹泻患者,以随机数字表法分为观察组与对照组,各40例。对照组给予益生菌治疗,观察组给予升阳益胃汤加减治疗。对比两组治疗3、7、14 d的腹泻症状、病情严重程度、营养状态及达营养目标量时间。结果 治疗7、14 d,观察组大便次数、大便总量分别为(2.34±0.51)次/d、(206.34±22.16)g与(2.15±0.46)次/d、(175.35±23.16)g,少于对照组的(3.95±0.46)次/d、(287.55±29.57)g与(3.65±0.57)次/d、(235.67±30.59)g,会阴皮肤评估量表(PAT)评分别为(5.10±0.18)、(4.82±1.06)分,低于对照组的(6.32±0.22)、(6.02±1.16)分,差异有统计学意义(P<0.05)。治疗7、14 d,观察组格拉斯昏迷评分量表(GCS)评分分别为(11.20±1.33)、(13.26±1.57)分,高于对照组的(9.77±1.02)、(11.59±1.62)分,急性生理与慢性健康评分(APACHEⅡ)分别为(13.12±1.20)、(9.88±1.62)分,低于对照组的(15.12±1.26)、(13.64±1.57)分,差异有统计学意义(P<0.05)。观察组达营养目标量时间(4.15±0.62)d短于对照组的(6.77±0.72)d,差异有统计学意义(P<0.05)。治疗3 d,两组血红蛋白(Hb)、白蛋白(ALB)、总胆固醇(TC)、淋巴细胞计数对比,差异无统计学意义(P>0.05);治疗7、14 d,观察组Hb、ALB、淋巴细胞计数分别为(121.54±7.22)g/L、(35.46±3.26)g/L、(1.58±0.21)×10^(9)/L与(132.56±8.16)g/L、(37.25±3.61)g/L、(1.63±0.23)×10^(9)/L,高于对照组的(110.62±6.42)g/L、(32.05±3.17)g/L、(1.46±0.25)×10^(9)/L与(111.87±9.23)g/L、(32.34±3.26)g/L、(1.48±0.31)×10^(9)/L, TC分别为(2.28±0.40)、(2.01±0.54)mmol/L,低于对照组的(2.55±0.39)、(2.25±0.51)mmol/L,差异有统计学意义(P<0.05)。结论 予以神经重症营养性腹泻患者升阳益胃汤加减治疗效果确切,可推广。Objective To analyze the clinical effect of addition and subtraction of Shengyang Yiwei Decoction on nutritional diarrhea in patients with severe neurological diseases.Methods 80 patients with nutritional diarrhea due to severe neurological diseases were included in an observation group and a control group under the random number table,with 40 cases in each group.The control group was treated with probiotics,and the observation group was treated with addition and subtraction of Shengyang Yiwei Decoction.The diarrhea symptoms,severity,nutritional status and time to reach the nutritional target of the two groups were compared on the 3rd,7th and 14th day of treatment.Results On the 7th and 14th day of treatment,the number of stool and total amount of stool in the observation group were(2.34±0.51)times/d,(206.34±22.16)g and(2.15±0.46)times/d,(175.35±23.16)g,which were less than those of(3.95±0.46)times/d,(287.55±29.57)g and(3.65±0.57)times/d,(235.67±30.59)g in the control group;the perineal assessment tool(PAT)scores were(5.10±0.18)and(4.82±1.06)points in the observation group,which were lower than those of(6.32±0.22)and(6.02±1.16)points in the control group;the differences were statistically significant(P<0.05).On the 7th and 14th day of treatment,the scores of the Glasgow coma scale(GCS)in the observation group were(11.20±1.33)and(13.26±1.57)points,which were higher than those of(9.77±1.02)and(11.59±1.62)points in the control group;the acute physiology and chronic health evaluationⅡ(APACHEⅡ)score were(13.12±1.20)and(9.88±1.62)points in the observation group,which were lower than those of(15.12±1.26)and(13.64±1.57)points in the control group;the differences were statistically significant(P<0.05).The time to reach the nutritional target of(4.15±0.62)d in the observation group was shorter than that of(6.77±0.72)d in the control group,and the difference was statistically significant(P<0.05).On the 3rd day of treatment,there was no statistically significant difference in hemoglobin(Hb),albumin(
